Overview
Licensed Practical Nurse working in Long Term Care on a travel assignment in Kremmling, Colorado. Start date 08/24/2026 for 13 weeks. Earn $1,118–$1,148 per week with 32 guaranteed hours per week. Shift is Nights, 12-hour shifts in a Critical Access Hospital setting, with potential coverage in Granby, CO.

Responsibilities

Assess, plan, implement, and coordinate care for SCU and Skilled Nursing/Extended care patients
Administer medications and treatments per orders; monitor patient status and document findings
Collaborate with RNs and the broader care team; communicate changes in patient condition promptly
Provide patient and family education; assist with discharge planning and continuity of care
Maintain safety, infection control, and adherence to facility policies and procedures
Participate in on-call coverage as required; respond to patient needs during night shifts and escalate as needed

Requirements

Current LPN license (state verification per job notes)
Ability to work night shifts, 12-hour blocks
On-call availability required (2 shifts per 6 weeks)
Experience or comfort providing care in a skilled nursing/extended care setting
Willingness to travel for a temporary assignment; 13-week contract length
Ability to work in a hospital-based, longer-term care environment

Housing assistance: Shared furnished employee housing may be available based on capacity at a reduced inclusive rate of $54/hr; no pets, partners, or smoking allowed

On-call coverage: Required (two shifts every six weeks)
