LPN – Extended Care / Skilled Nursing | Travel Contract | CO

Job Details

Position: Licensed Practical Nurse(LPN) – Skilled Nursing facility (SNF)
Location: Kremmling, CO 80459
Contract Length: 13 Weeks
Start Date: ASAP
Schedule: Night Shifts - Variable Days 12 hour shifts
Shift: 6:00 PM – 6:00 AM (Including Holidays)
EMR: EPIC
Employment Type: Travel Contract
Number of Openings: 1

Responsibilities of an LPN – Extended Care / Skilled Nursing / SCU (as per the job description)

Assess patients' physical, emotional, and psychosocial needs.
Plan, implement, and coordinate individualized patient care.
Provide direct nursing care to patients in Skilled Nursing, Extended Care, and Special Care Unit (SCU) settings.
Monitor patient conditions and promptly report changes to the RN or provider.
Administer prescribed medications and treatments according to physician orders.
Perform routine nursing procedures, including wound care, dressing changes, and catheter care.
Monitor and document vital signs, pain levels, intake/output, and patient responses to treatment.
Maintain accurate and timely documentation in EPIC electronic medical records.
Assist with activities of daily living (ADLs) while promoting patient independence.
Collaborate with physicians, registered nurses, therapists, and interdisciplinary healthcare teams.
Educate patients and families regarding treatments, medications, and discharge planning.
Respond appropriately to medical emergencies and changes in patient condition.
Follow infection prevention, patient safety, and quality care standards.
Participate in patient assessments and care plan updates.
Ensure continuity of care during shift handoffs and interdisciplinary communication.
Float to Granby Med-Surg unit as needed to support patient care.
Participate in on-call rotations as required (2 shifts every 6 weeks).
Work night shifts, weekends, holidays, overtime, and variable schedules as required.
Maintain compliance with facility policies, state regulations, and nursing standards.
Provide compassionate, patient-centered care while preserving patient dignity and confidentiality.

Job Requirements

Active Colorado LPN License (current and unrestricted).
Minimum 3 years of LPN experience.
Experience in Med-Surg, Long-Term Care (LTC), Skilled Nursing, Extended Care, or Special Care Unit (SCU).
BLS/BCLS certification required.
ACLS certification required.
PALS certification required.
ENPC certification required.
TNCC certification required.
Experience using EPIC electronic medical record system.
Ability to work 12-hour night shifts with variable days.
Ability to participate in on-call rotations and work overtime when needed.
Willingness to float to another facility/unit as required.
Ability to work weekends and holidays.
Strong assessment, communication, critical thinking, and patient care skills.

Pay: $1,790.00 - $1,816.00 per week
