Home Health Operations Coordinator

Position Overview

Optima Home Health Care is seeking an organized, dependable, and detail-oriented Home Health Operations Coordinator to support the daily operations of our growing home health agency.

This position plays an important role in keeping patient care organized and moving efficiently. The Operations Coordinator will primarily be responsible for clinician scheduling, physician order management, supply coordination, and assisting with patient intake as needed.

The ideal candidate is highly organized, comfortable communicating with clinicians, physician offices, patients, and referral sources, and able to manage multiple priorities in a fast-paced healthcare environment.

Key Responsibilities

Scheduling & Coordination

Coordinate and maintain patient visit schedules for RN, PT, OT, ST, MSW, and HHA staff.
Assign visits based on clinician availability, patient location, discipline, and scheduling requirements.
Communicate schedule changes with clinicians and office staff.
Monitor unassigned, missed, rescheduled, and outstanding visits.
Assist with coordinating Start of Care (SOC) visits and new admissions.
Help ensure ordered visit frequencies are appropriately scheduled.
Identify scheduling conflicts and work proactively to resolve them.

Physician Order Management

Track outstanding physician orders and certifications.
Send orders to physician offices for signature.
Follow up on unsigned or incomplete orders.
Maintain accurate documentation of order status and follow-up attempts.
Upload and organize completed orders in the appropriate system.
Communicate outstanding order issues to clinical and administrative leadership.
Help ensure required documentation is obtained in a timely manner.

Supply Management

Coordinate patient and office supply requests.
Maintain inventory of commonly used supplies.
Track supply usage and reorder supplies as needed.
Communicate with clinicians regarding patient-specific supply needs.
Help prevent unnecessary or duplicate supply orders.
Maintain organized supply records and inventory.

Intake Support

Assist the intake department as needed with new referrals.
Enter patient demographic and referral information.
Review referrals for missing documentation.
Assist with obtaining physician orders and supporting documents.
Communicate with referral sources, physician offices, patients, and caregivers.
Help move accepted referrals toward scheduling and Start of Care.

General Operations

Answer and route telephone calls professionally.
Maintain accurate electronic records.
Assist clinicians with scheduling and administrative questions.
Support communication between intake, scheduling, clinical management, and field clinicians.
Protect patient confidentiality and maintain HIPAA compliance.
Complete other administrative and operational duties as assigned.

Qualifications

High school diploma or equivalent required.
Previous healthcare office experience preferred.
Home health experience strongly preferred.
Experience with scheduling, intake, medical records, physician orders, or healthcare coordination is highly desirable.
Strong computer and data-entry skills.
Excellent telephone and professional communication skills.
Strong organizational and time-management abilities.
High attention to detail.
Ability to prioritize multiple tasks and meet deadlines.
Comfortable communicating with patients, caregivers, clinicians, physicians, hospitals, and other healthcare organizations.
Ability to work independently while functioning effectively as part of a team.
Knowledge of HIPAA and patient confidentiality requirements.

Experience with Medicare-certified home health, EMR systems, OASIS-related workflows, physician orders, insurance verification, or referral management is a plus.

The successful candidate should be able to recognize problems before they become urgent—for example, an unassigned visit, an outstanding physician order, a missing referral document, or supplies that need to be reordered—and take appropriate action.

This position is ideal for someone who is organized, proactive, professional, dependable, and comfortable working in a fast-paced home health environment.

Compensation

$16.00–$18.00 per hour

Starting compensation will be based on relevant home health, healthcare scheduling, intake, and administrative experience.
