Overview
Join our dynamic community in-home mental health team as a Licensed Clinical Social Worker/Therapist OR an LMSW in training OR use your Bachelors in Social work to provide case management. Cambian has several positions in the social work field avaliable at this time, apply and talk through your career goals further to see if its a good fit!

If you are an clinical applicant and serve within this role, you will provide expert mental health assessments, evidence-based psychotherapy, and compassionate case management to promote healing and resilience. You will work within a multidisciplinary environment dedicated to delivering comprehensive behavioral health and social services, ensuring clients receive personalized, culturally sensitive care. This, commission base, paid position offers an exciting opportunity to utilize your clinical expertise while advancing your professional growth in a supportive setting. Bachelors or Master's degree with no license in social work, but hold a degree in this field are eligible for case management opportunities.

Duties for Clinical Positions (LCSW, LMSW):

Conduct thorough mental health assessments and diagnostic evaluations using established clinical standards and ICD coding systems (ICD-10, ICD-9).
Develop and implement individualized treatment plans incorporating evidence-based practices such as cognitive behavioral therapy, psychodynamic therapy, applied behavior analysis, and trauma-informed care.
Provide individual, and group therapy sessions focusing on mental health interventions for conditions including PTSD, substance use disorders, , developmental disabilities, and geriatrics.
Engage in crisis intervention and crisis management to support clients experiencing acute emotional distress or behavioral challenges.
Document all clinical encounters accurately within Electronic Medical Records (EMR/EHR) systems while adhering to HIPAA regulations and medical documentation standards.
Collaborate with multidisciplinary teams including medical professionals, educators in special education settings, social workers, and community organizations to coordinate comprehensive patient care.
Collect data for clinical research projects aimed at improving mental health services and outcomes; participate in quality assurance activities related to clinical documentation standards.

Case Manager Position Duties:

Complete therapeutic referrals, both internally and externally, to help a client obtain needed services to address identified needs and achieve goals specified in the Treatment Plan of Care.
Complete monitoring, making recommendations for necessary adjustments and follow-up activities to ensure the Treatment Plan of Care is implemented and adequately addresses the client’s needs as necessary. Annual monitoring should be conducted to determine services are met such as:
o Services are being furnished in accordance with the Treatment Plan of Care.
o Services in the Treatment Plan of Care are adequate.
o Changes in the needs or status of the client are reflected in the Treatment Plan of Care.
Monitoring and follow-up activities include making necessary adjustments in the Treatment Plan of Care and service arrangements with providers.
Complete collateral contacts as needed to ensure client's access to therapeutic services.
Conducting scheduled and as needed functional assessment to determine if additional interventions and/or services are needed.
Complete all documentation in accordance with agency standards including documentation in an electronic health record; this includes, but is not limited to, content expectations and timeliness standards.

Clinical Professionals Should Aquire These Skills

Proven expertise in psychotherapy modalities such as harm-reduction, recovery-oriented, person-centered, trauma-informed, cognitive behavioral therapy, psychodynamic therapy, behavioral therapy, and addiction counseling.
Strong knowledge of mental health assessments, psychiatric patient care, diagnosis coding (ICD-10/ICD-9), CPT coding, and medical terminology.
Experience working with adults with developmental disabilities, substance use disorders, and individuals from diverse cultural backgrounds.
Skilled in crisis intervention strategies and managing complex cases involving trauma or substance use disorders.
Proficiency with EMR/EHR systems for documentation and data collection; familiarity with HIPAA compliance policies.
Ability to perform comprehensive patient assessments including intake procedures and case management activities.
Excellent communication skills for client engagement, social work practices, and interdisciplinary collaboration within hospital or community care settings.
