Job Overview

Join our dynamic healthcare team as a Certified Home Health Aide (CHHA), where your compassionate care and dedication will make a meaningful difference in the lives of our clients. As a vital member of our caregiving team, you will deliver personalized support to individuals across various settings, including senior care, hospice, and individuals with disabilities. Responsibilities

Provide direct patient care by assisting with ADLs such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
Monitor and document patient vital signs, medical records, and progress using Electronic Health Record (EHR) systems while ensuring compliance with home healthcare regulations and HIPAA standards
Observe patients closely for changes in condition, including dementia, Alzheimer's disease, or other cognitive impairments; report findings promptly to supervising nurses or physicians
Administer medications and perform catheter care following prescribed protocols and safety procedures
Support patients with meal preparation, feeding, laundry, cleaning, and other household tasks to promote a safe and comfortable environment
Assist with specialized care needs such as hospice support, end-of-life care, memory care, and working with individuals with developmental disabilities or disabilities
Use medical terminology accurately during documentation and communication; demonstrate proficiency in vital signs measurement, CPR/First Aid/Basic Life Support (BLS), and other emergency response skills

Requirements

Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) in accordance with state regulations
Proven experience in assisted living facilities, nursing homes, or home healthcare environments; experience with geriatrics and long-term care preferred
At least one year experience as a Home Health Aide and successful completion of competency evaluation
Ability to perform tasks involving physical activity, including heavy lifting, extensive bending, and standing
Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
Strong knowledge of home health procedures including medication administration, hospice care, dementia care, and working with patients who have Alzheimer's or other memory impairments
Ability to lift heavy objects safely and perform physically demanding tasks such as patient transfers using Hoyer lifts or similar equipment
Excellent patient observation skills coupled with compassionate caregiving abilities; understanding of HIPAA compliance and confidentiality standards
Current CPR/BLS certification; first aid training is a plus
Flexibility to work live-in shifts or community-based care settings; reliable transportation is preferred
