Position Title: Guardianship Case Manager
Reports To: Guardianship Program Director
Status: Full Time; Exempt
Purpose of Position: The Guardianship Case Manager supports vulnerable older adults who have been determined by the courts to need guardianship protection by ensuring all health and well-being needs of clients are met.

Responsibilities:
- Conduct monthly face to face visits to assess well-being, inquire of needs and advocate for older adults with cognitive impairments
- Serve on-call 24/7 for client emergencies during specified times
- Document case notes of all client activities in a detailed, accurate, timely manner per contract guidelines and in compliance with HIPAA, Texas Medical Privacy Act, and Texas Identify Theft Enforcement and Protocol Act
- Coordinate timely completion of required reports, including but not limited to Annual Reports, Service Plans, and Bill of Rights reviews
- Make all decisions for guardianship clients in a client-centered manner by encouraging self-determination, by use of substituted judgment or best interest standards, as appropriate
- Knowledge of long-term care regulations and willingness to facilitate inquiries of poor care to ensure the well-being of highly vulnerable individuals, including assessment for least restrictive alternatives and make arrangements for alternative placement when indicated
- Coordinate and attend medical appointments, gain knowledge of clients’ diagnoses, work with medical professionals to determine treatment options, advocate for best care practices, and make all medical decisions for clients
- Work with our Guardianship Benefits Coordinator or other financial responsible parties to apply for and maintain entitled supports and services and to monitor financial records and benefits status on-going to ensure accuracy
- Work with families and friends of guardianship clients who may have challenging family dynamics to provide education and direction, as well as build working relationships to facilitate positive and healthy connections
- Testify in Court for routine and complicated guardianship matters and knowledge of court procedures
- Interact routinely with the probate courts, county courts, court personnel, clerks, and attorneys
- Report any concern for abuse, neglect, or exploitation to the proper authorities including the Department of Aging and Disability Services, Adult Protective Services, Office of Inspector General, Long Term Care Ombudsman, and/or local law enforcement
- Developing a working knowledge of, and remaining current on, local, state and national issues involving guardianship, and engaging in systems advocacy related to those issues, when appropriate
- Assist with benefits coordination including application / renewal of Medicaid, Medicare, Social Security, VA benefits, etc.
- Assist with volunteer training coordination
- Attend regular team meetings, all-staff meetings, and in-service training sessions
- Ability to perform work duties independently but also equally available to collaborate with colleagues on client, program and agency activities.
- Integrate technology to remain accountable in the field by maintaining a shared calendar and remaining available for on-going communication during work hours through various electronic modes
- Commitment to ensuring and maintaining accuracy of client information in Salesforce database
- Comfortable and collaborative in accepting supervisory direction pertaining to decision making and other case manager activities
- Ensure all responsibilities of case manager are performed within time frames and requirements of Texas Minimum Standards for Guardianship Services, TGA and NGA Standards, the Texas Estates Code and any contracts or grants Senior Citizens of Greater Dallas, Inc. d/b/a The Senior Source has for guardianship services
- Perform other duties that may be assigned
Qualifications:
- Bachelor’s or Master’s degree in social work from an accredited college or university and a minimum of one year of full time experience (with Master’s degree) or two years of full time experience (with Bachelor’s degree) in direct social service work. (LMSW preferred)
- Ability to pass a state certification test and become a certified guardian within a year of hire date.
- Experience working older adults, and experience working with volunteers, a plus.
- Strong organizational skills and attention to detail.
- Strong interpersonal and communication skills with a desire for continuous learning.
- Excellent technology skills, with proficiency in Salesforce or other CRM platform required.
- Ability to carry out assignments and projects without detailed instructions.
- Ability to thrive in a fast-paced, deadline-driven environment.
- Bilingual in English and Spanish preferred.
Must have reliable transportation, as this job requires extensive local travel.
