Education/Experience/Training:
ØHigh School Diploma, GED or HSE;

ØA minimum of two (2) years working in social services or similar experience in a like field and experience completing discharge/treatment planning and documentation;
ØRecruitment and selection processes will involve a written essay, oral interview, criminal background check and psychological evaluation.
Certifications:
ØPossess or be eligible for a valid Montana driver’s license;
ØIT and CJIN Security Awareness Training Certificates required upon assignment of such training;
ØFirst Aid and CPR certification required within one (1) year of employment;
ØAbility to obtain additional licenses and/or certifications as required by the position.
Sheriff’s Office Employment Statement:
Any individual with a felony conviction, a domestic violence conviction or outstanding warrants is ineligible for consideration for employment by the CCSO.

This is a temporary grant-funded position with funding until June 30, 2027. This grant may or may not be funded beyond this date.
JOB SUMMARY
The Community Care Coordinator (Coordinator) is a civilian position at the Cascade County Sheriff’s Office (CCSO) whose primary function is to conduct, coordinate and implement discharge planning for inmates in the Cascade County Adult Detention Center (ADC) who are receiving and/or are in need of mental health and substance use disorder services as indicated by the treatment team. The Coordinator will also review all new inmate admissions upon entry into the ADC and work with the treatment team to determine mental health service needs;, maintain a procedure and assist inmates who need follow up in order to continue essential medications, chronic care, and/or their treatment plans by seeking appointments with community resources/health providers, assist inmates with needed services admission criteria, eligibility and enrollment requirements, assist inmates in obtaining identification cards/drivers licensing, when indicated, provide inmates with assistance in submitting application as well as refer them to assigned agency and correctional facility programs, act as an educational liaison and provide consultation to medical personnel regarding discharge planning processes, applicable regulations and accreditation standards, communicate all discharge planning issues with members of the treatment team, communicate all discharge planning with the inmate, inpatient, and outpatient community team, and participates in and makes CCSO policy development recommendations related to the discharge programs. The Coordinator provides support, guidance and training to and for staff and performs other duties as required or assigned.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
Manages and oversees the discharge programs in collaboration with the treatment team and the ADC Captain in accordance with all applicable regulatory authority and CCSO policies, procedures, guidelines and standards; Coordinates and oversees the overall operation and implementation of the discharge program activities and makes CCSO policy development recommendations related to the discharge programs; Assesses and evaluates CCSO staff as it applies to discharge programs and provides such assessment and evaluation to the ADC Captain and/or the Records Division Manager; Responsible for conducting, coordinating and implementing discharge planning for inmates who are receiving and/or are in need of mental health and substance use disorder services; Develops and maintains a procedure for compliance for releasing prescriptions and addressing recurring prescriptions following release; Reviews and screens all new inmate admissions upon entry into the ADC to assess and determine their mental health and substance use disorder service needs and meet with inmates to complete a discharge planning evaluation to determine all discharge planning needs as indicated by treatment team; Develops a service plan with inmates describing screening results, referrals made, planned and actual services received, inmate goals and progress toward them and discharge plans; Based upon screening and assessment results and other relevant information, provide inmates with relevant referrals for services (behavioral health, and substance use disorder education prevention and treatment, primary care, transitional housing, employment food assistance, etc.); Assists inmates in the ADC who need follow up in order to continue essential medications, chronic care, and/or their treatment plan by seeking appointments with willing community resources/health providers; Assists inmates with understanding of needed services admission criteria, eligibility, and enrollment requirements, when applicable; Assists inmates in the ADC in obtaining identification cards/drivers licensing, when indicated; Provides inmates with assistance in submitting applications as well as referrals to assigned agency and correctional facility programs; Acts as educational liaison and provides consultation with medical personnel regarding discharge planning processes, applicable regulations, and accreditation standards; Uses data and information obtained through work functions to identify discharge programs needs and training; Communicates all discharge planning issues with all members of the treatment team; Communicates all discharge planning with the inmate, inpatient, and outpatient community team; Develops and maintains current information about community-based services, what they offer, availability of services and contact information for continuity of support and establish and maintain professional and effective relationships with them to provide appropriate resources and on-going support for inmates; Ensures regulatory compliance with HIPAA, CCSO procedures, plans and protocols, mandatory reporting requirements, adherence to the Administrative Rules of Montana (ARM) and other applicable rules, regulations, standards and guidelines; Regularly informs and educates CCSO staff regarding any changes in the discharge program requirements, schedules, developments and presentations; Responsible for creating and maintaining relationships with community agencies providing services to persons with mental illness; Develops and utilizes a discharge resource book identifying community resources, i.e. social service resources available in the community; Demonstrates accurate and timely documentation in accordance with established guidelines; Prepares and submits statistical data, monthly activity and annual reports as directed; Interacts and communicates in an effective, professional and respectful manner with co-workers, supervisors, other county employees, the treatment team, medical community providers, other agency personnel and citizens of varied and diverse social, economic and cultural backgrounds; Interact and communicate with inmates in an empathic, effective, professional and respectful manner while respecting their confidentiality and privacy; Establishes and maintains professional and respectful relationships with co-workers, supervisors, other county employees, the treatment team, medical community providers, other agency personnel and the general public; Ensures compliance with all applicable regulatory standards; Performs detailed work with a high degree of accuracy, organization and coding results, adheres to established policies and procedures and works in a professional and effective manner both independently and in a group setting; May appear and testify in court proceedings; Prepares clear and concise reports in accordance with established policies and procedures; Strictly adheres to and complies with all applicable confidentiality policies, procedures, rules, standards and guidelines, including the Health Insurance and Portability and Accountability Act (HIPAA), and all applicable data security policies, procedures, rules, standards and guidelines; Attends trainings, meetings, conferences, seminars and workshops as requested and authorized; Performs work when called out on weekends, afternoons and nights; Performs other duties as required or assigned.
