Position Summary
Sun River Health & Rehab is seeking compassionate and skilled Registered Nurses (RNs) to join our nursing team. Full-time and part-time opportunities are available.
As an RN, you will provide clinical leadership and direct resident care while ensuring the highest standards of nursing practice are maintained. You will play a critical role in assessing resident needs, administering treatments, coordinating care, supervising nursing staff, and partnering with physicians, families, and interdisciplinary team members to deliver exceptional resident-centered care.

Essential Duties and ResponsibilitiesResident Care & Clinical Assessment
- Provide direct nursing care to residents in accordance with physician orders, individualized care plans, and nursing standards of practice.
- Perform comprehensive assessments and ongoing evaluations of resident health status.
- Monitor residents for changes in condition and initiate appropriate nursing interventions.
- Deliver person-centered care that respects each resident's preferences, goals, rights, and dignity.
- Participate in care planning and collaborate with the interdisciplinary team to achieve positive resident outcomes.
Medication & Treatment Administration
- Administer medications, treatments, and therapeutic interventions safely and accurately.
- Document medications, treatments, observations, and resident responses in the electronic medical record.
- Monitor for medication effectiveness, side effects, and adverse reactions.
- Enter physician orders and ensure timely implementation of prescribed treatments.
- Maintain medication carts, treatment carts, and medication storage areas in compliance with facility policies and regulations.
Clinical Leadership & Supervision
- Supervise and support Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and other nursing personnel.
- Delegate tasks appropriately in accordance with the Nurse Practice Act and facility policies.
- Provide guidance, mentorship, and clinical support to team members during assigned shifts.
- Assist in maintaining a positive, collaborative, and resident-focused care environment.
Documentation & Regulatory Compliance
- Complete assessments, progress notes, incident reports, care plan updates, and required clinical documentation accurately and timely.
- Maintain complete and compliant medical records for assigned residents.
- Participate in quality assurance initiatives, audits, surveys, and regulatory inspections.
- Ensure compliance with federal, state, and local regulations governing skilled nursing facilities.
Resident Admissions & Discharges
- Assist with resident admissions, readmissions, and discharges.
- Review admission orders, complete initial assessments, and coordinate care transitions.
- Orient new residents and families to the facility and care processes.
- Prepare discharge instructions, medication information, and education materials as appropriate.
Communication & Care Coordination
- Participate in shift-to-shift report to ensure continuity of care.
- Complete controlled substance counts and reconciliation during shift changes.
- Communicate effectively with physicians, nurse practitioners, families, therapists, pharmacy providers, laboratories, and other healthcare partners.
- Notify providers and family representatives of significant changes in resident condition.
Safety & Quality Improvement
- Initiate and complete incident reports and required follow-up documentation.
- Identify clinical concerns and implement interventions to promote resident safety and quality outcomes.
- Follow infection prevention and control practices.
- Participate in facility quality improvement programs and performance initiatives.
Additional Responsibilities
- Collect laboratory specimens and coordinate testing as ordered.
- Order medications and nursing supplies as needed.
- Assist in maintaining clean and organized nursing stations, work areas, and supply rooms.
- Attend staff meetings, required training, and continuing education programs.
- Perform additional duties as assigned by nursing leadership.
QualificationsRequired
- Graduate of an accredited nursing program.
- Current, unrestricted Registered Nurse (RN) license in the state of practice.
- Current Basic Life Support (BLS) certification or ability to obtain upon hire.
- Ability to successfully complete all required pre-employment screenings, including criminal background checks.
- Strong clinical assessment, critical thinking, and decision-making skills.
- Excellent communication, leadership, organizational, and time management abilities.
- Proficiency with electronic medical records and healthcare technology systems.
Preferred
- Previous nursing experience in a skilled nursing facility, rehabilitation center, hospital, long-term care facility, or similar healthcare setting.
- Experience caring for geriatric, rehabilitation, dementia, and medically complex populations.
- Prior supervisory or charge nurse experience.
Physical Requirements
- Ability to stand, walk, bend, and move throughout the facility for extended periods.
- Ability to lift, transfer, push, or carry items exceeding 30 pounds when necessary.
- Ability to respond quickly to resident care needs and emergency situations.
- Ability to work effectively in a fast-paced healthcare environment.
- Ability to provide care to residents with dementia, chronic illnesses, mental health conditions, and physical disabilities.

Pay: $44.00 - $47.00 per hour
