Full Job Description

Job Summary
We are seeking a dedicated and compassionate Case Manager to join our dynamic team. In this vital role, you will coordinate and deliver comprehensive support services to individuals and families facing complex challenges, like disabilities.

Duties

Conduct thorough assessments to identify clients’ needs across behavioral health, child welfare, and workforce development sectors.
Develop tailored case plans that incorporate crisis management strategies, motivational interviewing techniques, and resource linkage.
Provide crisis intervention support during urgent situations, ensuring immediate safety and stabilization for clients experiencing mental health or behavioral crises.
Coordinate services related to Section 8 housing programs, Medicare assistance, and childcare support to promote stability and self-sufficiency.
Mentor clients through goal-setting activities, empowering them with skills for independent living and personal growth.
Collaborate with community partners, healthcare providers, and social service agencies to facilitate comprehensive care delivery.
Maintain detailed documentation of case activities, progress notes, and service plans in compliance with organizational policies.

Requirements

Proven experience in social work, behavioral health, or related fields with a focus on crisis intervention and drug addiction.
Strong knowledge of crisis management techniques and behavioral health principles.
Demonstrated ability to supervise or mentor team members or clients effectively.
Familiarity with child welfare systems, Section 8 housing programs, Medicare processes, and workforce development initiatives.
Excellent communication skills with the ability to motivate clients using motivational interviewing strategies.
Experience working with diverse populations, including children requiring childcare support or mentoring services.
Ability to handle high-pressure situations with professionalism and empathy while maintaining clear documentation standards.

Pay: From $19.00 per hour
