Hillcrest Health & Living is looking for a compassionate, dependable, and service-focused Personal Care Assistant / Medication Aide to join our assisted living team at Hillcrest Silver Ridge in Gretna, NE.

As a Personal Care Assistant / Medication Aide, you will play an important role in providing personalized care, medication support, and daily assistance based on each resident’s individual needs, preferences, and Resident Service Agreement.
What You’ll Do
As a Personal Care Assistant / Medication Aide, you will work closely with residents, nurses, and other team members to provide medication support, personal care, and assistance with daily living while encouraging independence and maintaining dignity.
You will:
- Administer medications to residents according to the five rights of medication administration and accurately document medications provided.
- Monitor residents following medication administration and communicate concerns or changes to the appropriate licensed healthcare professional.
- Assist residents with activities of daily living, including bathing, dressing, grooming, hygiene, and other personal-care needs.
- Support residents with adaptive devices such as hearing aids, glasses, and dentures.
- Monitor residents and communicate changes in health, behavior, or needs to the appropriate nurse or clinical leader.
- Obtain and document vital signs as ordered, including temperature, pulse, blood pressure, respirations, and weight.
- Promptly report vital signs that fall outside established parameters to the Nurse Manager or Director of Health Services.
- Assist residents to and from meals and help ensure everyone is accounted for during dining.
- Provide meal and dining support based on individual resident needs.
- Understand when and how to access emergency medical services when needed.
- Communicate resident concerns and needs to nurses and incoming or outgoing team members to support continuity of care.
- Assist with resident laundry, including personal clothing and linens.
- Complete required documentation in the electronic medical record.
- Help maintain clean, comfortable, and organized resident living areas.
- Assist with general cleaning responsibilities such as making beds, removing trash, organizing, and vacuuming as needed.
- Support scheduled resident activities and encourage participation based on individual interests and abilities.
- Build positive relationships with residents and families through compassionate, respectful, and dependable care.
- Assist with additional resident-care and community needs as assigned.
The position combines medication administration, personal care, vital-sign monitoring, dining assistance, resident communication, and daily living support.

Ideal candidates will have:
- Previous experience as a Medication Aide, Personal Care Assistant, caregiver, CNA, Nurse Tech, or similar direct-care role preferred.
- Experience working with aging adults in assisted living, memory support, senior living, long-term care, or a similar setting preferred.
- Strong attention to detail with medication administration and documentation.
- A compassionate and respectful approach to personal care.
- Strong communication and relationship-building skills with residents, families, nurses, and team members.
- Ability to recognize and promptly communicate changes in a resident’s condition or behavior.
- Strong attention to detail with medications, vital signs, documentation, and resident-service plans.
- Ability to support residents while encouraging as much independence as safely possible.
- Dependability and consistent follow-through.
- Strong organization and the ability to manage multiple resident needs.
- A calm, flexible, and professional approach in stressful or changing situations.
- Comfort using electronic medication and documentation systems.
- A team-first attitude and willingness to assist wherever needed.
Education & Credentials
- High school diploma or equivalent preferred.
- Medication Aide coursework preferred at the time of hire.
- Nebraska Medication Aide certification preferred at the time of hire.
- Previous experience providing personal care to aging adults in assisted living or memory support preferred.

Physical & Other Requirements
- Must be able to speak, read, and write English.
- Must be able to independently lift up to 35 pounds.
- Must be able to perform repetitive sitting, bending, standing, walking, reaching, lifting, and movement as needed.
- Must be able to safely assist residents with personal care and activities of daily living.
- Must be flexible, calm, and professional when working with residents or families in stressful or emotional situations.
- Must be able to work in a fast-paced environment with strong attention to detail.
- Must be able to utilize medication systems, electronic documentation systems, technology, and available resources efficiently and effectively.
- Possible exposure to communicable diseases and bodily fluids.
