Full Job Description

Job Overview

Professional Case-Management Services of America (PCSA) currently has a full-time Support Coordinator (Case Manager) Trainer position open in the Tucker/Atlanta Area.

RESPONSIBILITIES

1. Provide ancillary supervision and support to newly hired Support Coordinators as assigned

2. Ensure that newly hired support coordinators (as assigned) facilitate all activities related to gathering information to be included in the support plan.

3. Ensure that newly hired support coordinators are conducting monthly site visits and recording visits accurately on the site monitoring form.

4. Provide support to active Support Coordinators

5. Provide support to Regional Management team.

Trainers maybe be assigned a small caseload and will perform the following activities for individuals assigned to them while not conducting training for mentees.

6. Assist the individual and family in developing annual budget and implementing the ISP.

7. Oversee the person’s participation in the Services, support and activities delineated within the ISP.

8. Complete all documentation which includes but is not limited to: CIRs, RCRS, DMA7, discharges, and transfers based on individual support needs.

KNOWLEDGE, SKILLS, AND ABILITIES

Knowledge of social, economic, health or rehabilitative services objectives.
Knowledge of methods of compiling, organizing and analyzing data.
Knowledge of problem-solving techniques.
Ability to plan and develop programs or program components.
Ability to utilize problem-solving techniques.
Ability to evaluate and monitor service delivery and implement corrective action plans.
Ability to provide technical assistance regarding program and client issues.
Ability to supervise people.
Ability to determine work priorities, assign work, and ensure proper completion of work assignments.
Ability to communicate effectively.
Ability to establish and maintain effective working relationships with others.
Ability to understand and apply applicable rules, regulations, policies and procedures.

MINIMUM QUALIFICATIONS

A bachelor’s degree or equivalent in (social sciences)
1 Year experience working with IDD Individuals

Pay: $42,000.00 - $45,000.00 per year
