Carrier Mills Nursing and Rehabilitation is seeking an experienced and motivated MDS Nurse Manager (RN or LPN) to join our leadership team. This vital role is responsible for coordinating the MDS process, supporting quality resident outcomes, maximizing reimbursement accuracy, and ensuring compliance with federal and state regulations.

Schedule

Full-Time
Monday–Friday
8:00 AM – 4:30 PM
Participation in the management on-call rotation is required.

Compensation

Registered Nurse (RN): $33.00–$35.00 per hour (Based on Experience)

Licensed Practical Nurse (LPN): $29.00–$31.00 per hour (Based on Experience)

$10,000 Sign-On Bonus

Paid out over one year.

Position Responsibilities

Coordinate and oversee the MDS assessment process.
Complete and submit accurate, timely MDS assessments in accordance with CMS guidelines.
Lead interdisciplinary care plan meetings and collaborate with department leaders.
Monitor case mix and reimbursement opportunities.
Maintain compliance with federal and state regulations.
Support quality measures, survey readiness, and regulatory compliance initiatives.
Work closely with nursing leadership to promote outstanding resident outcomes and documentation accuracy.

Qualifications

Current Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required.
Previous MDS and care planning experience required.
Strong knowledge of Medicare, Medicaid, and reimbursement systems preferred.
Excellent communication, organizational, and leadership skills.
Strong attention to detail with the ability to prioritize multiple responsibilities.
Long-term care experience preferred.
