Full Job Description

Job Type Full-time

Description

This position will serve as the primary contact for people in the community seeking information and referral, care coordination, and case management for elders in the community aging in place, and for Jewish elders living in supportive, assisted living, or other non-independent housing. This may include coordination with out-of-area adults, elder law attorneys, and facility social workers as needed.

Provide Health Care Navigation services, including and not limited to:

Develop and implement care plans for senior clients, including assessment of physical, emotional, psychological, and social needs. Provide health care navigation services.
Collaborate and coordinate care for clients with them and extended family.
Provide education and counseling to clients, families, and caregivers regarding health care needs and available resources.
Develop and implement strategies to resolve care-related issues.
Assist with referrals to appropriate community services and resources.
Plan for acute hospital discharge needs, including arrangements for follow-up care and home health services, in collaboration with hospital social workers.
Advocate for clients to ensure their physical and emotional needs are met.
Provide crisis intervention when needed
Monitor progress of clients and provide regular updates to families, physicians, and other care providers.
Coordinate educational sessions, to Including securing location and speakers.
Connect and act as a liaison between JFS and local synagogue staff, including but not limited to identification of elders in need of supportive services, and community needs.
Coordinate and lead community-based support groups related to topics pertinent to the Jewish Community, including but not limited to grief and loss groups, caregiver support, living with a partner with Dementia, etc.
Provide onsite consultations and personal needs assessments to community elders during food pantry hours and for other JFS events.
Conduct routine (at least twice yearly after intake) home visits with Kosher Meals on Wheels (KMOW) clients to assess their needs.
Maintain monthly senior newsletter providing news, events, and senior related information to the local community.
Oversee financial assistance program (Back on Track, or BOT) to seniors in need of specific financial assistance, as well as to other clients as identified by JFS program staff.
Data collection related to services provided and assistance in reporting to grant Organization as needed.

Requirements

Knowledge of the aging process and its physical, psychological, and social implications.

Knowledge of the health care system and available resources for the elderly.
Understanding of the roles and responsibilities of a geriatric case manager and health care navigator.
Ability to assess patient needs and develop comprehensive care plans.
Ability to develop and implement strategies to facilitate patient care objectives.
Ability to communicate effectively with clients, family members, healthcare providers, and other stakeholders.
Ability to coordinate with other professional staff to ensure timely, efficient, and cost-effective care.
Strong problem-solving skills.
Knowledge of applicable laws, regulations, and standards of practice related to senior care.
Proficiency in computer systems, applications and technology related to case management.
Knowledge of the Greater Harrisburg Jewish Community and its customs and culture.

The Senior Services Case Manager will play a critical role in the well-being of aging individuals and their families. Empathy and strong communication skills are essential for effective case management.

You must have the ability to explain medical and social services in an understandable way. In addition, you should possess excellent problem-solving skills, be organized, and have the ability to manage multiple tasks simultaneously. You must be familiar with available resources in the community and have a strong understanding of Medicare, Medicaid, Social Security, and other government programs.

With these skills, you can help aging individuals, and their families navigate their healthcare and financial needs, while optimizing their quality of life.
