Job Title: Dietitian / Nutritionist

Department: +MEDRITE Concierge – Chronic Care Management (CCM)
Reports To: Director of Chronic Care Management / Clinical Leadership
Employment Type: Full-Time / Part-Time /
Location: in-Office / Field-Based

Position Summary

The Dietician/Nutritionist plays a critical role in supporting patients enrolled in +MEDRITE Concierge’s Chronic Care Management (CCM) program. This position focuses on delivering personalized nutrition care plans, ongoing dietary counseling, and education to patients managing chronic conditions such as diabetes, hypertension, obesity, cardiovascular disease, and other long-term health concerns.

The ideal candidate is patient-centered, proactive, and experienced in working with diverse populations, with a strong understanding of nutrition’s role in disease prevention and management.

Key Responsibilities

Conduct comprehensive nutritional assessments for CCM patients, including dietary habits, medical history, and lifestyle factors
Develop individualized, evidence-based nutrition care plans aligned with patient diagnoses and care goals
Provide ongoing counseling and education to support behavior change, adherence, and improved health outcomes
Collaborate with physicians, nurse practitioners, care coordinators, and other interdisciplinary team members to ensure integrated care delivery
Monitor patient progress and adjust nutrition plans as needed based on clinical outcomes and patient feedback
Document all patient interactions and care plans accurately in the EMR in accordance with CCM and regulatory guidelines
Support patients with chronic conditions including (but not limited to):
Diabetes and prediabetes
Hypertension
Hyperlipidemia
Obesity and weight management
Gastrointestinal disorders
Participate in care plan reviews and case conferences
Assist in developing educational materials, wellness programs, and preventative care initiatives
Ensure compliance with CMS guidelines and CCM program requirements

Qualifications:

Bachelor’s or Master’s degree in Nutrition, Dietetics, or a related field
Registered Dietitian Nutritionist (RDN) certification required
Active state licensure (where applicable)
Minimum of 1–3 years of clinical or chronic care management experience preferred
Experience with telehealth and/or remote patient engagement is a plus
Strong knowledge of evidence-based nutrition guidelines and chronic disease management
Excellent communication, motivational interviewing, and patient engagement skills
Proficiency with EMR systems and digital health tools
