We are actively seeking an experienced Residential Program Manager and a health and wellness cordinator in Delaware County PA who will be responsible for the supervision and oversight of Direct Support Professionals in their assigned homes.
The Residential Program Manager leads a team of DSPs to provide person-centered care tailored to meet the needs of individuals with intellectual and developmental disabilities. You will support your team with day-to-day operations and challenges to achieve our programmatic goals.
Responsibilities
● Lead and train your team to support programmatic initiatives, goals, daily routines, and the unique needs of individuals with intellectual and developmental disabilities
● Provide training on daily routines including cleaning, cooking established meal plans, feeding, medication administration, and maintaining personal hygiene
● Manage team performance through effective communication, training, performance management, staff meetings, and employee recognition
● Communicate with the families and guardians of individuals we support as needed
● Ensure individuals are supported per their ISP/PCP including medication management, meal planning, and behavior management
● Ensure program documentation and billable records are completed accurately and timely
● Assist with monitoring the finances of individuals ensuring purchases are approved and accounted for with documentation
● Monitor the health and medical needs of individuals and immediately report any concerns
● Manage relationships with the families and guardians of the individuals in our care
● Ensure residences and vehicles are clean, well maintained, and stocked with necessary supplies including groceries, medications, and cleaning supplies
● Cooperate with audits, inspections, and investigations
● May be required to assist with scheduling and filling shifts
● Additional tasks and responsibilities may be assigned
HEALTH AND WELLNESS CORDINATOR
The Health and Wellness Specialist plays a crucial role in ensuring that residents of the group home receive appropriate and timely medical care, maintain their health and well-being, and comply with regulatory requirements under Chapter 6400. Their responsibilities include:
1. Appointment Scheduling and Coordination.
· Schedule all medical, dental, and specialist appointments for residents, ensuring appointments are aligned with their individual needs and preferences as outlined in their Individual Support Plans (ISPs).
· Ensure residents are up to date on all annual and routine appointments, including physical exams, dental cleanings, vision checks, and any other preventive care services.
· Coordinate transportation to and from appointments, ensuring that residents arrive on time and with all necessary documentation or medical equipment.
· Communicate appointment details to staff, residents, and guardians in advance, including date, time, location, and preparation requirements.
2. Provider Identification and Management
· Identify and maintain a network of healthcare providers, including primary care physicians, specialists, dentists, and mental health professionals, who can meet the specific needs of the residents.
· Maintain an up-to-date list of all healthcare providers, including contact information, specialties, and insurance coverage, to streamline coordination and communication.
· Evaluate and select providers based on accessibility, insurance coverage, and the quality of care provided.
· Act as a liaison between the group home, healthcare providers, and family members or guardians to facilitate seamless communication and coordination.
3. Appointment Follow-Ups and Refusals
· Ensure follow-up appointments are scheduled as recommended by healthcare providers.
· Document and file any appointment refusals by completing a Refusal Form to record the reason and communicate the refusal to the appropriate team members.
· Collect and review all consultation forms after appointments to ensure recommendations are implemented and the forms are filed in residents' medical records.
· Communicate relevant follow-up information to staff, residents, and their guardians, including any changes to care routines or additional instructions.
4. Medication Management and Changes
· Collaborate with prescribing physicians and pharmacists to ensure proper medication administration, including changes in dosages, new prescriptions, or discontinuations.
· Update medication administration records (MARs) promptly and accurately to reflect any medication changes.
· Train direct support staff on medication changes, including potential side effects, proper administration, and monitoring for adverse reactions.
· Monitor residents for compliance with medication regimens and report any concerns to the prescribing provider.
5. Documentation and Compliance
· Maintain detailed and up-to-date records of all healthcare appointments, provider recommendations, and medical changes in compliance with Chapter 6400 regulations.
· Ensure all residents’ medical files include current health assessments, immunization records, and medication histories.
· Review and file all consultation forms received during medical appointments to ensure continuity of care.
· Prepare for audits and inspections by ensuring all health-related documentation is organized and accessible.
6. Health Advocacy
· Advocate for residents’ healthcare needs during appointments, ensuring their concerns are addressed and their rights are respected.
· Educate residents about their health conditions and treatment plans, promoting understanding and self-advocacy where possible.
· Address barriers to accessing healthcare, such as transportation or scheduling conflicts, to ensure timely and effective care.
7. Communication with Staff and Stakeholders
· Provide regular updates to staff, guardians, and other stakeholders regarding residents’ health and wellness.
· Train staff on recognizing symptoms that require medical attention and responding appropriately in medical emergencies.
· Act as a point of contact for any health-related inquiries or concerns from residents, families, or team members.
Minimum Requirements
● Bachelor’s Degree Required
● Complete all state and agency required training per state guidelines
● Valid driver’s license with a satisfactory driving record
● Experience supporting individuals with intellectual or developmental disabilities/Experience Supervising staff in this role a plus
● Knowledge of state regulatory agency operations and standards relevant to supporting individuals with intellectual or developmental disabilities
● Team oriented with demonstrated leadership experience
● Experience with problem solving against multiple priorities
● Ability to establish a comfortable and supportive relationship with individuals receiving supports
● Ability to exercise good judgment and remain calm in crisis situations
● Excellent verbal and written communication skills
● Excellent time management skills
Working Conditions
● Work in residential programs, and in the community
● Schedule may change due to business needs and may include evening and weekend hours
● May be required to be on-call in cooperation with other management staff
● May be required to transport individuals utilizing your own vehicle or company provided vehicles
● May be required to lift or move 25+ pounds
● May assist with wheelchair transfer of non-ambulatory individuals
● May be exposed to various medical conditions and communicable diseases
