About He Ala Hou O Ke Ola

He Ala Hou O Ke Ola, Inc. (HAHOKO) is a Honolulu-based 501(c)(3) nonprofit whose name means “A New Path of Life.” We are a faith-based community reintegration program serving men who are ready to transition into a life of freedom, serenity, and purpose. We support individuals who are experiencing or exiting homelessness, in recovery from substance use, and rebuilding their lives, through clean and sober housing, workforce development, and wraparound support services.

About the Role

As a Community Integration Services (CIS) Specialist, you will help Medicaid (Med-QUEST) members secure and maintain stable housing in the community. Through pre-tenancy and tenancy support, you will guide participants along the path from crisis to lasting housing stability—working one-on-one in homes and in the community, and providing direct support to residents in our residential and clean and sober housing (CSH) homes. This is a meaningful, people-first role for someone who wants to make a direct impact in the lives of our participants.

Key Responsibilities

Provide direct support to residents in HAHOKO’s residential and clean and sober housing (CSH) homes, promoting a safe, structured, and recovery-supportive living environment.
Support residents with daily living, house rules and routines, curfews, and adherence to program and sobriety expectations.
Conduct intake, needs assessments, and person-centered housing support planning with participants.
Provide pre-tenancy support: housing search, applications, obtaining documents, and preparing for move-in.
Provide tenancy support: coaching on tenant responsibilities, budgeting, and maintaining a stable tenancy.
Connect participants to community resources, benefits, healthcare, treatment, and employment services.
Build relationships with landlords and community partners to expand housing opportunities.
Accompany and transport participants to appointments and housing-related activities as needed.
Maintain accurate, timely case notes and documentation in compliance with Med-QUEST and CIS requirements.
Collaborate with the care team and report participant progress toward goals.

Qualifications

Passion for serving individuals experiencing homelessness, in recovery, or reentering the community.
High school diploma or GED required; associate or bachelor’s degree in social work, human services, or a related field preferred.
Experience in case management, direct support, peer support, or housing services preferred.
Strong interpersonal, communication, and organizational skills.
Valid Hawaiʻi driver’s license, clean driving record, and reliable transportation.
Ability to pass a criminal background check and required health clearances.
Basic computer skills for documentation and email; alignment with our faith-based, recovery-focused mission.

Schedule, Compensation & Benefits

Status: Full-time (40 hours/week).

Setting: Community-based fieldwork with office and facility duties on Oʻahu.

Pay: Commensurate with experience.
