Job Summary
We are seeking a highly skilled and dedicated Pharmacist to join our healthcare team. The ideal candidate will possess comprehensive knowledge of pharmacy operations, patient care, and medication management within various clinical settings. This role involves providing expert pharmaceutical services, ensuring medication safety, and supporting patient health outcomes through effective medication therapy management. The Pharmacist will collaborate with healthcare professionals to deliver exceptional patient care while adhering to regulatory standards and maintaining confidentiality under HIPAA guidelines.

Responsibilities

Dispense prescription medications accurately using pharmacy software and electronic health record (EHR) systems
Provide patient counseling on medication usage, potential side effects, and interactions
Conduct patient assessments to determine appropriate therapy plans, including immunizations and medication administration
Prepare compounded medications using aseptic techniques and ensure sterile procedures are followed
Manage inventory levels, order supplies, and ensure proper storage of pharmaceuticals
Perform clinical pharmacy services such as pain management, medical management, and medication reconciliation
Administer immunizations and IV infusions following proper protocols and safety procedures
Maintain compliance with HIPAA regulations by safeguarding patient information
Collaborate with healthcare teams in acute care, hospital, or LTC pharmacy settings to optimize patient outcomes
Assist in training pharmacy technicians and support staff to ensure efficient workflow

Skills

Extensive knowledge of LTC pharmacy, critical care, pediatrics, and acute care environments
Experience with pharmacy residency programs or clinical pharmacy experience preferred
Proficiency in pharmacy software, EMR systems, and EHR systems for accurate documentation and medication tracking
Strong understanding of physiology, anatomy, medical terminology, and pharmacology principles
Skilled in compounding medications and aseptic techniques for IV insertion and infusion therapy
Ability to perform patient service tasks including assessment, education, and immunizations
Familiarity with inventory management practices to ensure medication availability and safety
Experience with hospital-based pharmacy operations, pain management protocols, and medical management strategies
Knowledge of HIPAA regulations to ensure patient confidentiality and data security
Prior pharmacy technician experience is advantageous for supporting daily operations
