RN Case Manager (Outpatient/Inpatient)

Pay Rate: $70.00 - $77.00 Per Hour

Location: Tuba City, AZ 86045
Facility: Tuba City Regional Health Care Corporation
Contract Length: Contract Assignment
Schedule: Monday - Friday, 8:00 AM - 5:00 PM
Weekend Rotation & On-Call Required

Job Summary

Tuba City Regional Health Care Corporation is seeking an experienced RN Case Manager to coordinate and manage care for high-risk patients in both outpatient and inpatient settings. The Case Manager will work collaboratively with interdisciplinary teams to ensure quality, cost-effective healthcare while facilitating care transitions, discharge planning, utilization review, and resource coordination.

This position provides an excellent opportunity to work with diverse patient populations while supporting patients through complex healthcare and social challenges.

Position Responsibilities

Coordinate care transitions from admission through discharge
Conduct patient assessments and develop individualized care plans
Collaborate with physicians, advanced practice providers, nursing staff, and ancillary departments
Evaluate patients in the Emergency Department for medical necessity and care coordination needs
Facilitate safe discharge planning and outpatient follow-up services
Monitor patient progress and adjust care plans as needed
Coordinate community resources and support services for patients and families
Assist patients with chronic illness, behavioral health, substance abuse, and social service needs
Support utilization review and case management processes
Ensure continuity of care and regulatory compliance throughout the patient care journey

Requirements

Associate's or Bachelor's Degree in Nursing
Active and Unrestricted RN License in Any U.S. State or Territory
Current American Heart Association (AHA) BLS Certification
Minimum 3 Years of Clinical Nursing Experience
At Least 1 Year of Outpatient and/or Inpatient Experience
Knowledge of Case Management, Utilization Review, and Care Coordination Processes
Strong Communication, Assessment, and Critical Thinking Skills

Preferred Experience

Emergency Department Care Coordination
Utilization Review

Behavioral Health and Mental Health Coordination
High-Risk Patient Management
