ABOUT THE COMPANY

DHI365 is a rapidly growing healthcare technology and patient care organization focused on improving outcomes for patients through telehealth, Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Transitional Care Management (TCM), Behavioral Health Integration (BHI), and coordinated care services.

We currently serve patients throughout Pennsylvania and New Jersey and continue to expand our innovative care model nationwide.

POSITION OVERVIEW

DHI365 is seeking a Part-Time Telehealth Physician to join our growing clinical team.

This position is fully remote and focuses on providing high-quality virtual care through scheduled telehealth visits. The physician will work closely with care coordinators, clinical support staff, and remote monitoring teams to support patients enrolled in telehealth, RPM, CCM, TCM, and other care management programs.

The ideal candidate is clinically strong, patient-focused, comfortable with telehealth technology, and experienced in managing chronic conditions and ongoing patient care through virtual platforms.

KEY RESPONSIBILITIES

Telehealth Patient Care

Conduct virtual patient consultations using DHI365’s secure telehealth platform

Review patient medical history, current medications, allergies, symptoms, and available clinical information prior to and during patient visits

Evaluate, diagnose, and manage patient conditions within scope of practice and applicable regulations

Develop individualized care plans and provide recommendations for ongoing treatment and follow-up care

Remote Patient Monitoring & Chronic Care Management

Evaluate patient eligibility for Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Transitional Care Management (TCM), and other care coordination programs

Prescribe remote monitoring devices when clinically appropriate

Review RPM data, patient trends, alerts, and monitoring reports

Support chronic disease management through ongoing patient assessment and clinical intervention

Identify patients requiring additional care management services or specialist referrals

Patient Education & Care Coordination

Provide preventive care counseling, disease management education, and treatment guidance

Collaborate with care coordinators, clinical staff, specialists, pharmacies, and provider partners to support continuity of care

Participate in care planning and coordination efforts designed to improve patient outcomes

Documentation & Compliance

Maintain accurate, timely, and complete documentation within the electronic health record system

Document patient encounters, treatment plans, clinical recommendations, and follow-up instructions

Ensure compliance with all applicable federal, state, payer, telehealth, prescribing, privacy, and HIPAA requirements

QUALIFICATIONS

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school

Active and unrestricted Pennsylvania medical license required

Additional licensure in New Jersey and/or Delaware preferred

Board Certification in Internal Medicine, Family Medicine, or Geriatric Medicine required

Current DEA registration and applicable prescribing credentials

Strong clinical judgment and decision-making skills

Excellent communication and patient-centered virtual bedside manner

Comfortable using telehealth technology, electronic medical records, and remote clinical workflows

Ability to work independently while collaborating effectively with interdisciplinary care teams

PREFERRED EXPERIENCE

Previous telehealth experience strongly preferred

Experience with Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Transitional Care Management (TCM), and Behavioral Health Integration (BHI)

Experience caring for Medicare populations and patients with multiple chronic conditions

Familiarity with virtual care platforms, remote monitoring systems, and healthcare technology solutions

Experience supporting long-term care, home-based care, or population health programs preferred
