Full Job Description

Position Summary

Perma Homecare is looking for a compassionate, detail-oriented Registered Nurse to join our home care team. In this role, you will be the clinical anchor for a caseload of home care clients — completing assessments, building Plans of Care that actually reflect what a client needs, and directly supervising the STNAs and Home Health Aides in the field who bring those plans to life. You will work closely with clients, families, physicians, and PASSPORT case managers to make sure every client is safe, well-cared-for, and getting the outcomes we promise.

This is a leadership role for a nurse who wants to be more than a set of hands — someone who can spot problems before they become incidents, coach aides to excellence, and keep Perma Homecare survey-ready every single day.

Key Responsibilities

Client Assessments & Plan of Care Development

Complete initial and ongoing in-home nursing assessments for new and existing clients, including start-of-care, resumption-of-care, recertification, and change-in-condition assessments.
Develop, review, and update individualized Plans of Care that reflect each client’s medical needs, ADL/IADL needs, safety concerns, cognitive status, and personal preferences.
Coordinate with physicians, PASSPORT case managers, family members, and other providers to obtain orders, communicate changes, and ensure continuity of care.
Ensure all Plans of Care meet Ohio Department of Medicaid, Ohio Department of Health, and PASSPORT program requirements, and are documented completely and on time in the agency EMR.
Identify clients at risk for hospitalization, falls, skin breakdown, or medication issues, and implement preventive interventions.

STNA / HHA Supervision & 60-Day Visits

Complete required supervisory visits for STNAs and Home Health Aides on schedule — including on-site 60-day supervisory visits — and document each visit thoroughly.
Observe aide-client interaction, review task completion against the Plan of Care, and confirm the aide is delivering care safely, respectfully, and within scope.
Coach and re-train aides as needed. Escalate performance concerns to the Director of Nursing and document the intervention.
Verify that aide documentation — including EVV clock-in/out and visit notes — accurately reflects the care provided.
Serve as the clinical resource aides call when they see something concerning in the home. Respond promptly, document, and follow up.

Compliance, Documentation & Communication

Maintain compliant, complete, and timely clinical documentation in accordance with agency policy and Ohio Administrative Code.
Participate in Quality Assurance / Performance Improvement (QAPI) activities, chart audits, and survey preparation.
Communicate professionally and promptly with clients, families, aides, and referral sources.
Report suspected abuse, neglect, or exploitation immediately per Ohio mandatory reporting law.
Follow all Perma Homecare policies, including the Alcohol-Free Workplace, No-Smoking, Client Transportation, HIPAA, and Bloodborne Pathogens policies.

Required Qualifications

Current, unrestricted Registered Nurse (RN) license in the State of Ohio.
Minimum of two (2) years of nursing experience, with at least one (1) year in home care, home health, hospice, community health, or a related setting.
Working knowledge of Ohio Medicaid home care rules, PASSPORT program requirements, and Plan of Care development.
Current BLS/CPR certification.
Valid Ohio driver’s license, reliable transportation, and current auto insurance meeting Ohio minimum requirements. Willingness to travel throughout the service area for client visits.
Ability to pass a background check consistent with Ohio Administrative Code Rule 3701-60-09 and complete pre-employment drug screening.
Proficiency with electronic medical records (EMR) and standard office software.

Preferred Qualifications

BSN preferred.
Prior experience with a PASSPORT or MyCare Ohio waiver population.
Experience supervising STNAs, HHAs, or other direct-care staff.
Familiarity with PCG (Public Consulting Group) home care audits or ODM survey processes.

Exposure to residential home environments, including homes with pets, stairs, and variable conditions.
Some evening or weekend availability may be required for supervision or on-call rotation.
