ADON & MDS Coordinator

Lakewood Villa is looking for an experienced RN to join our leadership team as an ADON / MDS Coordinator.
This is a unique opportunity to step into a highly visible role with influence across both clinical operations and MDS. You'll work closely with nursing leadership, providers, therapy, residents, and families while helping drive quality outcomes, reimbursement success, and day-to-day clinical excellence throughout the building.

What You'll Be Doing
- Oversee MDS assessments, CAAs, care plans, and supporting documentation
- Manage Medicare and PDPM processes to support compliance, reimbursement accuracy, and resident outcomes
- Collaborate with nursing, therapy, providers, and interdisciplinary teams regarding resident care and status changes
- Support day-to-day nursing operations and serve as a resource to clinical staff
- Participate in care conferences, resident reviews, admissions, and discharge planning
- Monitor documentation practices and support regulatory compliance
- Assist with quality improvement initiatives and survey readiness
- Help identify opportunities to strengthen clinical systems and processes
What We're Looking For
- Active Colorado RN license
- Previous MDS experience required
- Strong knowledge of Medicare, PDPM, RAI process, and care planning
- Previous long-term care leadership experience preferred
- Strong communication, organizational, and clinical assessment skills
