Full Job Description

Physical Therapist Assistant (PTA) – Home Health

$60 Per Visit | Flexible Schedule | Chicagoland & Surrounding Suburbs

Metro Continued Care is a growing home health agency specializing in orthopedic and spine rehabilitation, including post-operative recovery and non-surgical orthopedic conditions and injuries. We are seeking compassionate, motivated Physical Therapist Assistants (PTAs) to provide high-quality, one-on-one care to patients in the comfort of their homes.

Our patient population includes individuals recovering from joint replacements, spine surgery, fractures, sports and musculoskeletal injuries, chronic orthopedic conditions, and other mobility-limiting diagnoses. If you enjoy working independently, building meaningful relationships with patients, and helping individuals regain strength, mobility, and independence, we'd love to hear from you.

Responsibilities

Provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist.
Provide skilled physical therapy interventions in accordance with the plan of care established by the supervising Physical Therapist for patients recovering from orthopedic surgery and those with non-surgical orthopedic injuries and conditions.
Instruct patients and caregivers on therapeutic exercises, gait training, balance training, mobility, fall prevention, and home safety.
Monitor patient response to treatment and communicate changes in condition to the supervising Physical Therapist.
Accurately complete electronic documentation in a timely manner.
Collaborate with physicians, therapists, nurses, and other members of the healthcare team to ensure excellent patient outcomes.

Qualifications

Graduate of an accredited Physical Therapist Assistant (PTA) program.
Current Illinois PTA license in good standing.
Current BLS/CPR certification.
Valid driver's license, reliable transportation, and current auto insurance.
Home health experience is preferred but not required.
Strong communication, organizational, and documentation skills.
Ability to work independently while maintaining excellent patient care.
