MiraSol Health, Inc. is an independent, community-based, hospice and palliative care non-profit organization, serving Beaufort and Jasper counties for more than 40 years. Established to give comfort, honor and dignity for end-of-life patients and their families through compassionate physical, emotional and spiritual care, regardless of their financial circumstances. We help our valued patients, and their families experience the fullness and dignity they deserve, even as they face the anxieties associated with the end of life.

MiraSol Health, Inc. is seeking a Registered Nurse to join us full-time, a weekend Friday-Monday schedule. The successful candidate will be responsible for patient care and coordination with the interdisciplinary team. RN duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s physical, emotional, and spiritual needs. Come enjoy manageable caseloads and the ability to provide quality care based on patient need. Previous hospice case management or home health experience is a plus but not required. We cover Beaufort, Jasper, Colleton & Hampton Counties. Dependable transportation is a must. We look forward to meeting you!

Responsibilities

Coordinate and provide care that is safe, timely, effective, efficient, and client-centered
Handle case assignments
Facilitate multiple care aspects (case coordination, information sharing, etc.)
Help patients make informed decisions by acting as their advocate
Develop effective working relations and cooperate with the interdisciplinary team throughout the case management process
Required documentation - must be comfortable using an iPad
Adhere to professional standards as outlined by protocols, rules and regulations

Skills

At least 1 year of experience as a RN
Effective communication skills
Excellent organizational and time management skills
Familiarity with professional and technical emerging knowledge
Problem solving skills and ability to multi-task
Compassion
Current RN license, BLS certification

Job Type: Full-time

Pay: $83,200.00 - $87,360.00 per year
