Job Summary
We are seeking a dedicated and highly skilled PRN Home Health Registered Nurse (RN) to provide compassionate, patient-centered care in the home setting. The ideal candidate will possess a strong clinical background, demonstrating expertise in diverse nursing practices, including complex assessments, medication administration, and wound care. This role offers flexibility and the opportunity to make a meaningful difference in patients' lives by delivering high-quality home health services tailored to individual needs. The PRN RN will collaborate closely with interdisciplinary teams to ensure comprehensive care delivery while maintaining compliance with all regulatory standards.

POSITION SUMMARY:

The Home Health RN is responsible for traveling to a patient’s home to administer their services and helping patients maintain their independence. Their duties include comprehensive medical assessments, medication administration, medical and post-operative support, patient and family education and care coordination. It is also the RN’s responsibility to educate the patient and the families or their care provider in assisting the patients on a daily basis.

ESSENTIAL JOB FUNCTIONS:

Conduct Comprehensive Assessments (using the OASIS tool), develop and implement Plan of Treatment
Collaborate with physicians, agency staff and contract services to coordinate patient care
Initiate appropriate referrals for other medical services when indicated
Perform various types of skilled nursing visits
Promote and maintain patient health through teaching and learning appropriate self-care techniques including but not limited to basic wound care management.
Document direct patient services in a timely manner in electronic medical records
Collaborate with management team and office staff to facilitate timely office processing of paperwork and patient information
Participate in agency quality improvement activities
Maintain the highest level of ethical standards
Other job duties as necessary with the approval of the clinical director.

REQUIRED SKILLS, ABILITIES AND KNOWLEDGE:

POSITION QUALIFICATIONS

Must be a Registered Nurse (RN) with the Illinois Department of Professional Regulation
Must have a minimum of two years of home health experience
Psychiatric Nursing experience a major advantage
Current CPR certification
Must have and maintain a valid IL driver’s license, automobile insurance coverage
Must be willing to travel to surrounding areas and patients’ homes
Ability to work independently with minimal supervision and time management abilities
Ability to successfully complete required background check
Excellent communication and reporting skills

Pay: $80.00 - $100.00 per hour
