As a Clinical Direct Support Professional, you will become a trusted mentor and advocate, providing meaningful support to individuals with intellectual and physical disabilities as they build independence, develop new skills, and participate fully in their home and community.

This position is part of a female model behavioral/medical home supporting long-term clients, offering the opportunity to build strong relationships, provide consistent support, and celebrate individual progress over time.

Tues.-Fri.- 3pm-11pm

Essential Responsibilities:
As a Direct Support Professional, you will:

Follow daily program schedules and complete assigned responsibilities as directed by the team and Program Manager.
Support individuals in achieving their personal goals by implementing service plans, including daily living skill development and individualized programming.
Ensure compliance with all Residential Standards and Regulations established by the Division of Mental Health and Developmental Services and The Moore Center Services, Inc.
Complete accurate and timely documentation, including progress notes, reports, and required records.
Communicate effectively with individuals served, clinical supervisors, team members, and other stakeholders to promote consistent and coordinated support.
Assist individuals with activities of daily living, including personal care, dressing, and other supports that promote independence and self-advocacy.
Promote the health, safety, and well-being of individuals in all assigned environments.
Support and monitor medication administration in accordance with agency policies, training, and individual needs.
Complete required behavior management training and consistently implement approved behavioral interventions and support plans.
Document behavioral supports and interventions as outlined in approved behavior and risk management plans.
Participate in ongoing training, team meetings, and professional development opportunities.
Accompany and support individuals during medical appointments and other community-based activities.
Provide safe transportation using agency-approved vehicles.
Remain on shift until appropriate coverage is available, as directed by the House Manager or designated Shift Leader.
Assist with the general upkeep, cleanliness, and maintenance of the residence, furnishings, and equipment.
Follow New Hampshire mandated reporting requirements for suspected abuse, neglect, or exploitation.
Perform other duties as assigned by the supervisor.

Requirements:

Qualifications:

Education and Experience:

Bachelor’s degree in Human Services or a related field, or equivalent combination of education and experience.
Minimum of 1 year of experience in human services required; 3 years preferred.
Valid driver’s license and access to a safe, reliable vehicle during scheduled work hours.
Automobile liability insurance coverage of $100,000/$300,000 required.
Successful completion of criminal, motor vehicle, and Bureau of Elderly and Adult Services (BEAS) background checks that meet agency standards.
Completion of required state training through Relias Learning or an equivalent program preferred.
Successful completion of Management of Aggressive Behaviors (MOAB) Part I and Part II training.
As a bona fide occupational qualification (BFOQ), Direct Support Professionals may be required to accompany individuals to religious ceremonies based on the individual's personal preferences.

Demonstrate flexibility with scheduling and daily responsibilities.
Possess proficiency in Microsoft Office applications, including Word, Excel, and Outlook, and the ability to learn new software systems.
