Full Job Description

Move Forward Housing Stability Case Manager - Temporary (State Voucher Expansion)

Founded in 1990, Move Forward is a nonprofit organization providing housing case management and mental health therapy services to youth in the western suburbs. Move Forward housing programs include case management, drop-in and outreach services, a scattered-site transitional living program, and the Schools to Housing Program. The majority of Move Forward participants identify with BIPOC and/or LGBTQIA+ communities and have faced housing instability or homelessness.

The Schools to Housing Program provides housing stabilization and homelessness prevention services for families with school-aged children through partnerships with Hennepin County and local school districts. This temporary position supports the expansion of the Bring It Home Rental Assistance Program, a State of Minnesota initiative that provides rental assistance to low-income families experiencing housing instability. Through this program, the Housing Stability Case Manager partners with Hennepin County to help eligible families access rental assistance, maintain stable housing, and improve long-term housing and educational outcomes. Priority is given to families with children experiencing the greatest housing need.

JOB SUMMARY

The Housing Stability Case Manager – Temporary (State Voucher Expansion) provides intensive housing stabilization case management to families experiencing or at imminent risk of homelessness. This position supports the expansion of the Schools to Housing Program through the State Family Housing Voucher Initiative and additional participating school districts in partnership with Hennepin County.

The Housing Stability Case Manager works directly with families to promote housing stability, coordinate housing assistance resources, support voucher-related services, conduct housing searches, and connect families to community resources that improve housing stability, school engagement, and long-term family well-being.

This is a temporary full-time position supporting increased voucher activity and expanded school district participation from August 1, 2026 through October 31, 2027, contingent upon contract funding.

DUTIES AND RESPONSIBILITIES

CASE MANAGEMENT

Develop and strengthen relationships with education systems to identify students/families at risk of experiencing homelessness.
Respond to referral sources promptly and collaborate with Hennepin County school navigators and school social workers.
Consistently meet with participants in office, school, community, and home settings.
Assist families with maintaining housing to help students remain within their school district and support school attendance and engagement.
Work collaboratively with participants to assess their strengths, successes, and challenges and create customized, participant-driven housing stability plans. Update and modify plans to reflect current goals, needs, and barriers to housing retention.
Utilize a person-centered and flexible approach in goal setting and review.
Display trauma-informed and compassionate communication with participants.
Work with participants seeking new housing to conduct a housing search, including teaching search skills, connecting families to rental assistance opportunities, and providing education about community resources.
Support participants in meeting and communicating with property managers, lease signing, and apartment set-up. Educate participants on tenant rights and responsibilities.
Facilitate skill development in areas such as budgeting, financial literacy and management, nutrition, meal preparation, household maintenance, mental health, and parenting.
Assist participants in maintaining or increasing income through benefits assistance or involvement in education, employment, or other income-building activities.
Provide basic needs assistance, information, and referrals, including food shelves, affordable childcare, and public benefits.
Provide resources, referrals, and connections to community partners, including legal resources, disability services, educational supports, and therapeutic services.
Provide ongoing monitoring and reporting of participant progress.
Assist eligible families in accessing and maintaining Bring It Home Rental Assistance and other housing assistance resources.
Complete eligibility verification and other required documentation in accordance with Hennepin County and program requirements.
Coordinate with Hennepin County staff regarding rental assistance administration, participant eligibility, and program requirements.
Provide housing stabilization services to families across participating school districts and support implementation of the Bring It Home Rental Assistance Program.

GENERAL RESPONSIBILITIES

Attend supervision meetings, staff meetings, required agency trainings, and county-sponsored trainings, implementation meetings, and case consultation opportunities as scheduled.
Attend case consultations with School Navigators, school staff, Hennepin County partners, and other community partners as appropriate.
Complete accurate and timely documentation in accordance with agency standards, funding contracts, Hennepin County requirements, and program guidelines.
Seek consultation, training, and feedback to advance skills in providing culturally responsive, cross-cultural, and trauma-informed services.
Demonstrate commitment to Move Forward's mission, vision, and values.
Engage in respectful communication and contribute to a positive team environment.
Organize, coordinate, track, and complete multiple tasks while adapting to changing priorities, program procedures, and funding requirements.
Maintain technical and practical knowledge essential to job functions, including ClientTrack, HDS, HMIS, Google Workspace, and other required systems.

QUALIFICATIONS:

REQUIRED

A minimum of 2 years case management, social services experience and/or a human services related Associate Degree or Bachelor’s Degree
Culturally specific experience working with people of varying ages, culture, gender, and other identities
Knowledge of child welfare systems, public and federal assistance programs, and other community resources
Excellent verbal and written communication skills, especially for client advocacy
High level of personal and professional integrity, including ability to protect and maintain confidentiality
Strong organizational and time management skills
Valid driver’s license and auto insurance
Access to a reliable vehicle and ability to maintain good driving record and insurance levels as directed by Move Forward, to fulfill job duties that require travel into the community.
Background check will be conducted

PREFERRED

Experience working with housing assistance programs, rental assistance, or housing voucher programs.
Knowledge of landlord engagement, housing navigation, and landlord-tenant practices.
Experience collaborating with school districts, county agencies, or public benefit systems.
Previous work with youth and families who have experienced housing instability preferred
Multilingual skill in Spanish, Somali, or Hmong a plus
