Full Job Description

Key Responsibilities

Clinical Care & Patient Support

Provide brief behavioral health consultations, including same-day (“warm handoff”) and scheduled visits

Deliver evidence-based interventions focused on behavior change and skill-building

Support patients with a wide range of concerns, including but not limited to:

Stress, anxiety, depression, and panic symptoms
Tobacco/nicotine cessation
Alcohol and substance use concerns
Sleep difficulties and insomnia
Chronic pain and chronic medical conditions
Diabetes and other health behavior-related conditions
Hypertension and lifestyle-related concerns
Attention and concentration difficulties
Grief, loss, and adjustment issues
Relationship and family stressors
Parenting strategies and behavioral concerns
Coping with discrimination, trauma, or domestic violence

Care Coordination & Collaboration

Collaborate closely with primary care providers, nurses, care managers, and clinic staff
Accept referrals from PCPs and team members and support patient self-referrals
Assist patients with medication adherence strategies (does not prescribe medications)
Support continuity of care and appropriate referrals to specialty behavioral health services when indicated

Patient-Centered Care

Explore patients’ values and priorities to guide treatment planning
Develop individualized, goal-oriented care plans focused on functional improvement
Empower patients to engage in meaningful daily activities, even when experiencing health challenges

Documentation & Quality

Document services in the electronic health record in accordance with clinic and regulatory standards
Participate in team meetings, case consultations, and quality improvement initiatives
Support population health and brief intervention workflows within primary care

Visit Structure

Typical scheduled visits 40mins with 20mins same-day (“warm handoff”) every hour
Some patients may be seen for a single-visit, while others may receive a short series of visits to learn and practice new skills

Population Served

Patients of all ages, including children, adolescents, adults, and older adults
Services provided across a wide range of medical and behavioral health conditions

Qualifications

Required

Master’s or doctoral degree in Psychology, Social Work, Marriage & Family Therapy, Counseling, or related behavioral health field
Licensure According to state requirements
Candidates are required to possess a current, full, and unrestricted LPC or LMSW license

Preferred

Experience in integrated primary care or fast-paced clinical environments
Strong skills in brief interventions, motivational interviewing, and behavioral activation
Ability to work collaboratively as part of a multidisciplinary health team

Position Summary

The Behavioral Health Consultant (BHC) provides brief, evidence-based behavioral health services within a primary care setting as part of an integrated care team. BHCs work collaboratively with primary care providers and clinic staff to support patients’ physical and emotional health through same-day consultations, scheduled visits, and population-based interventions. Services are focused on functional improvement, behavior change, and alignment with patients’ values and goals.
