Full Job Description

Vecino Health Centers (Vecino) is a Federally Qualified Health Center (FQHC) dedicated to providing comprehensive, high-quality healthcare services to underserved communities. The Community Care Specialist helps provide enabling services, including health promotion efforts for both patients and members of the community. This position enacts care coordination efforts according to programmatic priorities. The Community Care Specialist works in clinical and community settings; it involves collaborating with various community partners.

Essential Job Duties & Responsibilities

Enact patient recall campaigns for continuity of care and to close care gaps for priority groups. Specific annual targets are set based on previous years' benchmarks.
Utilize risk stratification and a caseload approach to care coordination duties.
Manage patient registries and population health caseloads for priority groups.
Participate in health promotion team efforts for both patients and members of the community.
Support team efforts related to patient experience. This includes participation in quarterly patient appreciation events.
Promote Vecino service lines among patients and community members.
Support community health (outreach) efforts and programming by providing screenings in community settings and staffing community events as needed.
Stay current on population health, disease prevention, place-based (community) models, and reproductive health best practices.
Other duties as assigned by supervisor.

Qualifications, Education, and Experience

Health-related certification or degree to perform health screenings, such as Medical Assistant. Licensed Vocational Nurse preferred.
Minimum of 1year of experience in healthcare or community settings.
Experience in care coordination preferred.
Strong analytical and problem-solving skills.
Experience in EHR and population health platforms.
Excellent customer service and interpersonal skills.
Impeccable work ethic.
Ability to work independently and in interdisciplinary teams.
Knowledge of healthcare delivery systems as well as health and human services.
Bilingual (English-Spanish) required.

Requirements

Ability to sit, stand, bend, and stoop for (long) periods of time.
Ability to exert up to 20 pounds of force occasionally/frequently.
Ability to work in a stressful or noisy environment.
Ability to work flexible hours, including some evening and weekend work.
Ability to respond and de-escalate emergency/crisis situations.
Have reliable transportation to perform job duties.

Pay: $46,000.00 - $53,000.00 per year
