The Harris Center for Mental Health and IDD (“The HARRIS CENTER”) is looking for a Wraparound Facilitator to join our team. Under the general direction of the practice manager or designated supervisor, this position is responsible for providing ongoing intensive case management services through Wraparound facilitation. Must be able to make effective assessments and interventions in situations involving mental health services provided to individuals. Ability to network with schools, juvenile probation and other social service agencies to facilitate services for youth. Responsible for coordination, planning and implementation of the wraparound plan as the individual plan of care. Ability to communicate with youth and their families to promote collaboration when creating wraparound plans based on Trauma Informed and Patient Centered Model requirements, consumer need, and performance contract requirements.
Your role in Action:
- Performs clinical assessments to determine service needs and progress with consumer treatment goals.
- Participates in formulation of the consumer wraparound plans and assures wraparound plan is written and authorized with an LPHA signature for consumers on caseload.
- Ensures all services provided meet eligibility criteria and are authorized in accordance with agency and regulatory guidelines.
- Provides skills training and psychosocial rehabilitation services that address the severe and persistent mental illness and symptom related problems that interfere with the individual’s functioning.
- Provides interventions in response to crisis to reduce symptoms and avoid admission to a more restrictive environment.
- Performs activities to assist consumers in gaining and coordinating access to necessary care and services based on individual needs.
- Provides training/education on the nature of mental illness, the importance of medications, and other medication related information as outlined in The Harris Center curricula.
- Monitors consumer progress with treatment goals
- Monitors current compliance with eligibility requirements for ongoing treatment (CANS / ANSA, financial assessment, current diagnosis) using the monthly SAI report and other available information.
- Meets divisional productivity standards as outlined in the mental health division’s operational guidelines.
- Performs related work as assigned. Adapts to changing work requirements.
- Demonstrates ethical conduct in all clinical and professional activities.
- Complies with agency policies and procedures accrediting/regulatory bodies and performance contract documentation requirements.
- Attends and participates in team meetings with the clinical team.
- Contributes to suicide care and prevention by embracing our values and applying a trauma-informed approach rooted in safety, trust, choice, collaboration, and empowerment.
What qualifications you will need:
Education:

Bachelor's Degree from four-year accredited college or university with a major in a social, behavioral, or human service field, including but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, and educational psychology.
Experience:

Prefer minimum of 1 year of experience working in the field of mental health.

Must maintain a valid Texas Driver's License and be insurable under Agency policy.

- Basic understanding of mental illness, common signs and symptoms and able to identify difficulties with coping in community settings.
- Strong communication skills, listening and interpersonal skills above average, written documentation is clear, concise, well- constructed and grammatically correct.

The Harris Center will leverage existing collaborations with local Independent School Districts (ISD) in the Precinct 2 area of Houston to organize five (5) colocation teams, addressing the mental and behavioral health needs of students within their academic environment, including Galena Park, Spring Branch, Pasadena, Alief, Goose creek, Aldine, and HISD (all districts with whom The Harris Center has a current memorandum of understanding). The Harris Center will work with the local precinct to identify specific campuses that need student mental health care. In addition to addressing acute needs, students and families will be referred for long-term support within our community-based clinics, including access to integrated health (physical/primary care). The Harris Center will have a goal of 150 students served in the Project.
