Full Job Description

Position Summary

The Registered Nurse Case Manager (RNCM)-Hospital Based GIP Admissions is responsible for providing on-site hospice admissions, care coordination, patient and family education, and discharge planning for patients admitted under a General Inpatient (GIP) hospice contract within hospital settings. This role is hospital based and requires the RNCM to remain on-site for the entire scheduled shift. The RNCM serves as the primary on -site clinical and administrative hospice representative during the shift, in coordination with hospice leadership, physicians, and the interdisciplinary group (IDG) ensures timely, compliant, and compassionate hospice admissions and transitions of care. Hospice eligibility certification and ongoing medical direction remain the responsibility of the hospice physician and Medical Director.

Work Schedule:

Three (3) shifts per week, consisting of twelve (12) hours per shift.
Shifts typically begin or end at 7 pm; schedules may be adjusted based on clinical needs and leadership discretion.
Due to the 3-day shift schedule, employees typically have extended periods off between scheduled workday; PTO requests should be coordinated in advance in accordance with organizational policy.

Core Responsibilities:

On -Site Hospital Coverage:

Remain on-site in the hospital lobby or designated waiting area for the entire scheduled shift in accordance with the GIP admissions contract.
Maintain professional presence and availability to hospital staff, patients, and families.
Respond to new admissions within 30 minutes to ensure compliance with hospital contractual requirements.

Patient Assessment & Admission

Perform comprehensive patient assessments and clinical evaluations to assess hospice eligibility and appropriateness for GIP level of care, in collaboration with the hospice physician.
Complete hospice admissions, including obtaining informed consent and required documentation.
Create and maintain patient charts in the electronic medical records (EMR).
Utilize approved language services, including the Language Line, to ensure effective communication with patients and families.

Patient & Family Education

Educate patients, families, and hospital staff on hospice philosophy, goals of care, and services provided.
Demonstrate compassion, empathy, and professionalism in all patient and family interactions.
Support goals-of-care discussions and address questions related to hospice services.

Discharge Planning & Care Coordination

Manage discharge planning for GIP patients, ensuring discharge as fast as reasonably possible unless clinically indicated otherwise.
Discharge timing is determined based on clinical appropriateness and physician direction.
Coordinate safe and timely discharge, including:
Placement planning, Setting up transportation, Ordering durable medical equipment (DME), Ordering medical supplies & Facilitating medication orders.
Ensure all arrangements support a safe transition of care.

Orders, Documentation, & System:

Order durable medical equipment (DME) and supplies per physician orders and organizational guidelines.
Facilitate medication orders in collaboration with physicians and pharmacy partners.
Complete Do Not Resuscitate (DNR) documentation and coordination required signatures.
Utilize Docusign for electronic consents and documentation as required.
Complete all documentation in accordance with organizational policies, QAPI standards, and regulatory requirements.

How to verify insurance benefits
How to create and manage patient charts in the EMR
How to use the Language Line and other approved interpreter services
How to complete DNR documentation and obtain required signatures
How to utilize Docusign
Knowledge of QAPI and hospice documentation standards

Basic Qualifications/Requirements:

Current, unrestricted Registered Nure (RN) license in the state of employment.
Must maintain eligibility to practice as a Registered Nurse in all assigned states, as applicable.
Minimum of two (2) years of hospice, hospital, or acute care nursing experience preferred.
Experience with hospice admissions and GIP level of care strongly preferred.
Strong assessment, documentation, and care coordination skills.
Excellent communication, organizational, and time management skills.
Ability to work independently and manage all hospice-related responsibilities during assigned shifts.
Applicants must be legally authorized to work in the United States.

Travel requirements: 5%

Salary is annualized for reference and paid semi-monthly in accordance with company payroll practices. Employment is at will.
