Patient Outreach Specialist/Population Health Coordinator

Location: Pasco/Hernando County, Florida

Gulf Coast Medical Center is seeking a compassionate, motivated, and patient-focused Patient Outreach Specialist/Population Health Coordinator to join our growing healthcare organization.

This position plays a vital role in connecting patients with the care they need by conducting outreach calls, scheduling appointments, closing care gaps, supporting preventive health initiatives, and assisting patients with chronic care and population health programs.

What You'll Do

Conduct outbound patient outreach calls for preventive care, quality initiatives, and chronic care programs
Schedule and confirm appointments for primary care, Annual Wellness Visits (AWVs), follow-up appointments, and preventive screenings
Assist with patient engagement and enrollment in care management programs, including APCM and CCM
Contact patients regarding care gaps, quality measures, health maintenance needs, and recommended screenings
Conduct outreach for AgeWise screenings, Annual Wellness Visits (AWVs), and preventive health initiatives.
Assist with outreach to patients recently discharged from the hospital to facilitate timely follow-up appointments.
Support quality improvement programs by contacting patients with outstanding care gaps, screenings, referrals, and chronic disease management needs.
Track outreach activities, patient engagement efforts, and completed quality initiatives using internal reports and dashboards.
Assist with payer-specific outreach initiatives including Medicare Advantage, WellMed, Humana, and other value-based care programs.
Review reports and work queues to identify patients requiring outreach
Educate patients regarding available healthcare services and resources
Document all patient interactions accurately in eClinicalWorks (eCW)
Coordinate with providers, nurses, referral coordinators, and office staff to support patient care
Assist with population health initiatives and quality improvement programs
Maintain HIPAA compliance and patient confidentiality
Meet established productivity, quality, and customer service standards

Qualifications

Required

High school diploma or equivalent
Excellent communication and telephone skills
Strong organizational and multitasking abilities
Ability to work independently and as part of a team
Professional and courteous customer service skills
Basic computer proficiency

Preferred

Medical office/Healthcare experience strongly preferred
Patient outreach experience
Experience with Medicare Advantage populations
Knowledge of quality measures, preventive screenings, and care gap closure initiatives
Experience working with reports, dashboards, and patient outreach work queues
Experience with population health, value-based care, or quality incentive programs
Appointment scheduling experience
Call center or customer service experience (e.g., 50-100 outreach attempts daily)
eClinicalWorks (eCW) experience
Population health or quality program experience
Bilingual English/Spanish
