Full Job Description

Laurel Canyon Dialysis LLC, a growing dialysis provider in the San Fernando Valley, is seeking an experienced and compassionate Clinical Manager to join our team. Our organization is built on strong relationships, accessible leadership, and a shared commitment to our patients and our people. Our Clinical Manager will work closely with leadership and the clinical team, with the opportunity to make a meaningful impact on patient care, staff development, and the overall success of the facility.

The Clinical Manager plays a key leadership role in ensuring safe, effective, and compassionate patient care. This position works closely with the Medical Director, nursing team, certified hemodialysis technicians, administrative staff, governing body, patients, and families to promote positive clinical outcomes and improve the overall dialysis experience.

We believe exceptional dialysis care goes beyond the treatment itself. Patient education, communication, compassion, and customer service are essential to helping patients feel informed, respected, and supported throughout their care. Our Clinical Manager will lead by example, developing a team that consistently delivers individualized care while maintaining the highest standards of clinical quality, patient safety, and professionalism.

Responsibilities

Leadership, Culture & Customer Service

· Lead the facility in accordance with the company's core values and commitment to exceptional patient care and service.

· Create a culture of compassion, teamwork, accountability, professionalism, and dedication to the well-being of patients.

· Recruit, hire, train, develop, and retain qualified employees while establishing clear expectations and accountability.

· Manage staffing, schedules, productivity, attendance, overtime, payroll, performance evaluations, coaching, and corrective action.

· Maintain a visible leadership presence and foster open, collaborative, and professional communication.

· Work directly with the Medical Director and clinical leadership on clinical, operational, and quality initiatives.

· Develop and maintain strong relationships with patients, families, physicians, hospitals, referral sources, Kaiser, HMOs, PPOs, ESRD Networks, vendors, and other key partners.

· Oversee patient grievances and concerns and ensure timely, professional resolution.

· Promote a positive patient experience and respond effectively to patient, employee, physician, and operational concerns.

Clinical, Quality & Regulatory Oversight

· Ensure the facility complies with applicable federal, state, CMS Conditions for Coverage for ESRD Facilities, DHS requirements, accreditation standards, OSHA requirements, and organizational policies.

· Maintain continuous survey and regulatory readiness through routine audits, mock surveys, monitoring, and corrective action.

· Partner with the Medical Director and clinical team to ensure safe, high-quality dialysis care and adherence to physician orders, clinical protocols, infection prevention practices, and patient safety standards.

· Monitor clinical performance and outcomes, including dialysis adequacy measures such as Kt/V and URR, vascular access management, hospitalization trends, incidents, and other applicable quality indicators.

· Ensure accurate and timely patient treatment documentation and data entry in the EMR/EHR.

· Oversee appropriate medication, treatment, equipment, and dialysis-related processes.

· Monitor dialysis water treatment systems, equipment maintenance, safety records, and applicable manufacturer and regulatory requirements.

· Ensure staff maintain required clinical competencies and participate in ongoing education and training.

· Lead and participate in QAPI activities, including review of clinical trends, incidents, grievances, root cause analyses, corrective actions, and performance improvement initiatives.

· Coordinate interdisciplinary meetings and care planning for complex patients and clinical concerns.

· Ensure required reports, records, submissions, logs, and documentation are accurate and completed timely.

· Participate in Governing Body, QAPI, staff, interdisciplinary, and other required meetings and ensure follow-up on identified action items.

Operations, Financial & Facility Management

· Oversee all day-to-day facility operations, including patient scheduling, admissions, referrals, patient flow, staffing, and resource allocation.

· Ensure the facility maintains a safe, clean, sanitary, organized, and professional environment.

· Oversee facility equipment, building maintenance, preventative maintenance, emergency preparedness, and coordination with Biomedical/Technical personnel.

· Manage medications, medical supplies, inventory, purchasing, and vendor relationships.

· Monitor facility financial performance, labor utilization, productivity, supply costs, and operating expenses.

· Identify and implement opportunities to improve efficiency, reduce waste, and strengthen financial performance without compromising patient care or safety.

· Maintain effective contractual, vendor, hospital, payer, and community relationships in collaboration with corporate leadership.

· Ensure the facility maintains appropriate records, licenses, certifications, permits, inspections, and operational documentation.

· Identify risks, investigate incidents, implement corrective actions, and escalate significant clinical, operational, financial, staffing, or regulatory concerns appropriately.

· Maintain confidentiality and security of protected health information and medical records.

· Follow all company policies, procedures, safety requirements, and applicable healthcare regulations.

· Perform other duties as assigned to support the successful operation of the facility.

Qualifications

· Current California RN license in good standing required.

· Bachelor’s degree preferred in Nursing

· 5+ years of Dialysis nursing experience required.

· 4+ years of progressive leadership or management experience in dialysis, renal care, or a related healthcare environment.

· Experience leading clinical operations across multiple facilities preferred.
