At the Home Centered Care Institute (HCCI), our mission is to increase access to home-based primary care for medically complex patients and their families. Every relationship you build helps healthcare organizations provide better care for those who need it most.

We're looking for someone who is ambitious, resourceful, and driven to make an impact. Someone who takes initiative, follows through with urgency, and is motivated by helping clients succeed while contributing to HCCI's continued growth.

What You'll Do

You'll serve as the trusted relationship manager for an assigned portfolio of clients while actively identifying opportunities to expand HCCI's impact.

In this role you will:

Build strong, lasting relationships with healthcare organizations.
Lead new client development and onboarding and ensure every engagement begins with a positive experience.
Conduct Strategic Business Reviews that strengthen partnerships and identify future opportunities.
Proactively identify opportunities for new clients, renewals, additional services, and client re-engagement.
Respond promptly to new inquiries and guide prospective clients toward the right HCCI solutions.
Develop a healthy pipeline of opportunities and consistently achieve revenue goals.
Collaborate closely with internal teams to ensure exceptional client experiences.
Serve as a trusted advisor who understands each client's goals and recommends solutions that create lasting value.
Connect regularly with HCCIntelligence™ Community members, ensure they are receiving the desired value needed to renew, and participate in scheduled virtual office hours as an opportunity to upsell on practice-level services.

Qualifications

Bachelor's degree or equivalent experience.
2–5 years of success in sales, business development, account management, client success, or another client-facing revenue role.
Demonstrated success developing business, retaining clients, or expanding existing accounts.
Excellent communication, relationship-building, and consultative selling skills.
Experience managing a CRM and maintaining an active pipeline.
Strong organizational skills with excellent follow-through.
Healthcare experience is a plus but not required.
