Patient Health Navigator (Bilingual English/Spanish Preferred)

Washington Health Institute (WHI) is seeking a compassionate, energetic, and mission-driven Patient Health Navigator to join our growing Community Health & Wellness team. This position plays a vital role in improving health outcomes by connecting individuals impacted by HIV, sexually transmitted infections (STIs), viral hepatitis, and other chronic health conditions to life-saving healthcare and supportive services.

Guided by WHI's Hi-V Model (Find, Test, Teach, Link, Keep), the Patient Health Navigator serves as a trusted advocate, educator, and care coordinator helping patients overcome barriers to care while promoting long-term health, stability, and wellness.

What You'll Do For Patient Navigation & Care Coordination

Serve as a trusted point of contact for patients throughout their healthcare journey.
Develop individualized care plans in collaboration with patients, providers, and interdisciplinary care teams.
Address social determinants of health by connecting patients with housing, transportation, food assistance, employment resources, behavioral health services, substance use treatment, and other community supports.
Assist patients with health insurance enrollment, Ryan White eligibility, and access to healthcare benefits.
Provide ongoing follow-up to promote engagement, treatment adherence, retention in care, and successful health outcomes.

HIV Prevention, Testing & Linkage to Care

Conduct rapid HIV, Hepatitis C, and STI testing in clinic and community outreach settings.
Provide pre- and post-test counseling, sexual health education, and risk-reduction counseling.
Ensure immediate linkage to HIV medical care for individuals newly diagnosed with HIV.
Connect eligible individuals to PrEP services and other biomedical HIV prevention resources.
Develop individualized Harm Risk Reduction Plans (HRRPs) to support healthier behaviors and long-term wellness.

Community Outreach & Health Education

Plan and participate in community outreach events, health fairs, mobile testing initiatives, and educational workshops throughout the Washington metropolitan area.
Develop, build and maintain strong partnerships with community-based organizations, healthcare providers, shelters, educational institutions, and faith-based organizations.
Deliver culturally responsive health education focused on HIV prevention, STI awareness, viral hepatitis, PrEP, safer sex practices, and wellness promotion.
Represent WHI professionally while strengthening community engagement and trust.

Documentation & Program Compliance

Complete Ryan White eligibility determinations at intake and annual recertification.
Maintain accurate, timely, and compliant documentation within Electronic Medical Records (EMR), eClinicalWorks (eCW), and CAREWare.
Track patient outcomes, referrals, linkage activities, and retention efforts.
Support monthly, quarterly, and annual grant reporting requirements.
Ensure compliance with HRSA Ryan White Program standards, organizational policies, and confidentiality requirements.

Team Collaboration

Participate in interdisciplinary case conferences, medical rounds, and care coordination meetings.
Collaborate with physicians, nurses, behavioral health clinicians, social workers, patient navigators, outreach specialists, and community partners to provide coordinated, patient-centered care.
Communicate effectively with both internal and external partners to ensure continuity of care.

QualificationsRequired

Bachelor's or Master's degree in Public Health, Social Work, Community Health, Human Services, or a related field.
Minimum of one (1) year of experience in patient navigation, outreach, case management, community health, HIV services, or a related healthcare role.
Strong interpersonal, communication, and relationship-building skills.
Experience working with underserved or vulnerable populations.
Proficiency with Microsoft Office and electronic documentation systems.
Ability to work independently while thriving within a collaborative team environment.

Preferred

Bilingual in English and Spanish.
Experience working within Ryan White HIV/AIDS Program services.
Experience conducting HIV, STI, and Hepatitis C testing.
Knowledge of CAREWare, eClinicalWorks (eCW), or other electronic health record systems.
Familiarity with the Washington DC metropolitan area healthcare and community resource networks.

Working Conditions

Full-time onsite position (Monday–Friday, 8:00 a.m. – 4:30 p.m.).
Flexibility to work occasional evenings and weekends for outreach events and community engagement activities.
Local weekly travel throughout Washington, DC, Maryland, and Northern Virginia serving communities on the Mobile Clinic Van .
Ability to lift outreach supplies (up to 25 pounds), stand for extended periods during community events, and operate computers and mobile technology throughout the workday.
