Agency Overview

Family Support Services of Indiana, LLC provides high-quality home and community-based services to individuals through Indiana’s Medicaid waiver programs, including Pathways for Aging, Health & Wellness (H&W), Traumatic Brain Injury (TBI), and the Community Integration and Habilitation (CIH) waiver administered by the Family and Social Services Administration (FSSA) and Bureau of Developmental Disabilities Services (BDDS). Our mission is to help families care for their loved ones with dignity, respect, and compassion. Whether assisting an older adult to remain safely at home or supporting an individual with developmental disabilities we are committed to making a meaningful difference.

Position Summary

The Program Coordinator plays a key role in ensuring smooth program operations, compliance, and outreach across Indiana’s waiver programs, including the service of Structured Family Caregiving under the multiple waivers. This position emphasizes relationship-building with community partners, families, and caregivers, while overseeing the on-boarding and support of the individuals and their providers. The Program Coordinator supports the delivery of person-centered, high-quality care and helps expand the agency’s presence in the community.

Key Responsibilities Program Coordination & Oversight

Coordinate and implement services under Indiana’s Pathways for Aging, H&W, TBI, and CIH waivers.
Monitor compliance with Indiana Administrative Code (460 IAC), DDRS/BDDS regulations, and Medicaid documentation standards.
Maintain accurate caregiver and participant records to ensure regulatory alignment.
Assist with internal audits, documentation reviews, and state compliance checks.

Community Outreach & Engagement

Represent the agency at community events, fairs, and professional networking opportunities to strengthen partnerships.
Build relationships with BDDS, Area Agencies on Aging, case managers, and local organizations to expand referral sources.
Develop and distribute outreach materials to raise awareness of services.
Serve as a liaison between families, caregivers, and community stakeholders.

Key Responsibilities:

● Conduct bi-weekly in-home or virtual visits to assess and understand the evolving needs of the participant and caregiver. Deliver content, strategies and tools related to the management of the participant’s needs and behaviors and the caregiver’s self-care needs as well as caregiver performance and participant satisfaction.

● Provide coaching, mentorship, and ongoing education to caregivers regarding daily caregiving routines, behavior management, safety practices, and person-centered planning.

● Coordinate with the RN to address medical concerns or health training needs.

● Assist caregivers in accessing additional community resources or support services.

● Ensure all documentation is completed accurately and submitted timely per state and agency requirements.

● Participate in care plan meetings and interdisciplinary team reviews.

● Monitor caregiver well-being and recommend respite or other supportive interventions as needed.

● Conduct at a minimum of quarterly home visits.

Qualifications

Bachelor’s degree preferred in human services, social work, psychology, nursing, or related field; OR 2+ years of experience in a similar role.
Knowledge of Indiana’s Medicaid HCBS waiver programs, including Pathways for Aging, H&W, TBI, and CIH. Familiarity with the service of Structured Family Caregiving.
Familiarity with BDDS and DDRS regulations, 460 IAC, and Medicaid compliance requirements.
Strong communication, organizational, and outreach skills.
Experience with provider on-boarding, training, or certification processes strongly preferred.
Reliable transportation, valid Indiana driver’s license, and willingness to travel within service areas.
Ability to pass criminal background checks and meet employment eligibility requirements.

Work Conditions

Hours: Full-time, 9AM – 5PM (flexibility required for occasional evening/weekend outreach events).
Location: Primarily office-based with travel for outreach and provider visits.
