Position Summary:
Under the supervision of the Licensed Clinical Social Worker, the Associate Psychotherapist/ACSW delivers patient-centered care that addresses mental health conditions in alignment with the Family Health Matters Community Health Center's mission. This role focuses on providing comprehensive support to individuals and families. To meet the pre-licensing accrual hours necessary for licensure in their related field under the jurisdiction of the Board of Behavioral Sciences, the Associate Psychotherapist/ACSW will receive supervised hours of experience from a Licensed Clinical Social Worker.
Schedule : This is a full-time position following an alternative work week schedule (4/10) The schedule days may include Saturdays.
Work Setting: In-clinic and at an off-site location such as a shelters, school, FRC or other community settings
Essential Duties and Responsibilities:
- Provides mental health services including individual and family therapy, crisis intervention for patients with mild to moderate conditions. Provides appropriate referrals for patients with severe mental illness. Treats patients of all ages including children, adolescents, adults and seniors.
- Receive referrals from FHM Providers and offer feedback to co-manage patients identified with health issues that warrant brief psychotherapy services.
- Psycho-education for patients and their support systems;
- Medication adherence counseling and disease management counseling;
- Motivational Interviewing to develop behavioral strategies aimed at symptom reduction;
- Brief Psychotherapeutic consisting of problem-solving skills, cognitive intervention aimed at modifying negative thinking and promoting self-efficacy;
- Self-Care Plan development and skills training to facilitate self-disease management,
- Improved coping, distress tolerance, stress reduction, and relaxation;
- Substance use/abuse evaluation & counseling, identification of maladaptive coping strategies, and development of harm reduction strategies utilizing SBIRT codes; MAT evaluations and
- Solution- focused, Cognitive Behavioral Therapy, Dialectic Behavior Therapy, Interpersonal Therapy, Problem Solving therapy, Trauma/Seeking Safety, Crisis Therapy, adolescent and child therapy, Brief couple and family psychotherapy skills, as needed
- Provides outreach and brief psychotherapeutic interventions to at risk youth and homeless population.
- Keeps accurate, up-to-date records in electronic health record, adhering to timely chart closure policies.
- Improve clinical outcome with high prevalence mental disorders.
- Adheres to mandatory reporting requirements.
- Participates in the peer review process for Behavioral Health Clinicians.
- Participates in in-person or virtual supervision sessions with clinical and administrative supervisors.
- May be assigned to work at a satellite clinic/site on a scheduled, temporary basis as needed.
- May perform other job-related duties as assigned.
Including skill development in:
- Consultation with PCP's to enhance understanding of the patient, provide decision support for primary care treatment planning and assist in the implementation.
- Coordination of Care – Provide consultation to and coordinate care of patients with primary care staff. Identify, triage, and advocate for patients needing specialty mental health services hour psychotherapy and more intensive mental health treatment services.
- Crisis Intervention – Be immediately available to the community health center staff during working hours.
Requirements and Qualifications:
Licensure and Certifications:
- Master’s degree from an accredited college or university, with a major in Social Work, Sociology, Psychology, Marriage and Family Counseling, or a related field.
- Currently registered and in good standing as an Associate Clinical Social Worker or Associate Marriage and Family Therapist by the California Board of Behavioral Sciences (BBS).
- Current BLS Certification required
Experience and Skills:
- Bilingual required English/Spanish
- Must have minimum 2 years of independent counseling experience required.
- 1 year Competency in providing brief cognitive and behavioral interventions to individuals, families, couples, and groups with identified behavioral health/substance abuse problems in a primary care setting.
- Ability to maintain client dignity and confidentiality.
- Ability to establish and maintain effective working relationships with others.
- Proficient with use of computers.
- Communicate effectively in writing and orally.
- Excellent interpersonal, communication and organizational skills.
