Care Manager (Private Geriatric Care Management)

Guardian Senior Solutions | Orange County, CA (field-based + remote) Compensation: Base + performance incentive

The role in one sentence

You are the private social worker a family wishes the hospital had given them — sitting at the kitchen table, sorting out what's actually needed, and staying with them until it's handled.

How we work

Care comes first. Always, and completely.

When a family calls us, the first conversation is about the person, not the money. What's happening. What's changed. What the client can still do and what they can't. Who's been carrying the load and how close they are to breaking. You assess, you build a plan, and you get care in place — in-home support, a community, a hospice referral, whatever the situation actually calls for. You see it through.

Only after the care picture is settled and stable does anyone at Guardian raise the question of how to pay for it, and only for the families who need help with that question. Plenty won't. They'll get a plan, get their care coordinated, and be on their way, and that is a complete and successful outcome here.

We are function across five verticals — care coordination, real estate, mortgage, insurance, and life settlements — so when a family does hit a funding wall, we have real answers instead of a shrug. But that capability sits behind the care work, not in front of it. You will never be asked to open with it, steer toward it, or bring it up before a family is ready.

Everyone gets a plan

Not every family who calls us is going to be a private-pay client, and we don't treat that as a wasted call. If the right answer is a county program or a nonprofit, that's the answer, and you give it to them clearly and specifically — with a name, a phone number, and what to ask for.

You'll be expected to refer families to:

Council on Aging – Southern California — HICAP for unbiased Medicare counseling, the Long-Term Care Ombudsman program, and the Answers resource guide
Caregiver Resource Center OC — respite, caregiver counseling, and family support through St. Jude
OC Office on Aging — the Area Agency on Aging Information & Assistance line and resource directory
Age Well Senior Services — South County transportation, meals, and case management
IHSS through the OC Social Services Agency, Medi-Cal, Meals on Wheels OC, and Adult Protective Services when the situation calls for it

We tell families up front what we do, what it costs them, and how we're paid. Then we give them a plan. Every time, whether or not there's a dollar in it for us.

What you'll do

Conduct in-home and bedside assessments: functional status, cognition, safety, caregiver capacity, family dynamics
Build written care plans with real timelines, real cost ranges, and a clear next step
Coordinate and monitor in-home care, assisted living, memory care, and skilled nursing placements — including the follow-through after move-in, which is where most agencies disappear
Attend care conferences, discharge planning meetings, and physician appointments when a family needs an advocate in the room
Connect families to county and nonprofit resources whenever those are the right fit
Recognize when a family's real constraint is funding, and — once care is handled — bring in our home equity and insurance specialists at the right moment
Build referral relationships with hospital case managers, SNF discharge planners, elder law attorneys, private fiduciaries, and physician groups across Orange County
Deliver required client disclosures accurately and completely, and document them
Manage your caseload in our CRM

What we're looking for

Required

Clinical or social work credential: LCSW, LMSW, MSW, RN, or equivalent geriatric care management experience
3+ years working directly with older adults and their families — case management, discharge planning, hospice, or private care management
Real command of the Orange County care landscape: communities, agencies, county programs, cost structures, waitlists, who is good and who isn't
The specific temperament this job requires: enough patience to sit with a family member who needs an hour to get to what they're actually afraid of, and enough discipline to still run a full caseload. Both. Neither one alone is enough
Working literacy in how families fund care — long-term care insurance, VA Aid & Attendance, Medi-Cal eligibility basics, home equity. You don't need to be an expert on day one, but you need to be curious about the money, because the money is what decides most of these cases
Reliable transportation, valid CA driver's license, comfortable in homes and hospitals

Preferred

CMC, CCM, C-ASWCM, or ALCA membership
CDP or dementia-specific training
Familiarity with reverse mortgage, HECM, or home equity strategies for care funding
Existing referral network in South OC
