Full Job Description

Available Shift:

MHC

Monday (PM), Wednesday (PM), Thursday (PM), Friday (AM), Saturday (PM)

Job Responsibilities/Duties

Responsibilities:

Client Care & Safety

Conduct 30-minute client safety and wellness rounds throughout each shift
Provide continuous one-to-one observation for clients needing heightened safety precautions based on clinical recommendation
Serve as the primary frontline staff member for all client questions, concerns, and day-to-day needs
Build positive relationships with clients while maintaining professional boundaries
Support implementation of clients’ individualized treatment plans as directed by the clinical team
Utilize de-escalation techniques to resolve client concerns and behavioral issues whenever possible, and escalate unresolved concerns to clinical or operations leadership
Recognize signs that a client is in crisis; use verbal de-escalation and supportive communication to help stabilize the situation, conduct SAFE-T, ensure the client’s immediate safety, and notify clinical staff when additional intervention is needed
Encourage client participation in scheduled group sessions and weekend outings
Facilitate new client orientation by helping clients acclimate to the facility, addressing questions and concerns, and supporting a smooth transition into the residential houses
Complete new admit intakes and gather all information needed

Medication Management

Complete and document a controlled medication count through the company’s EMR or EHR system at the beginning of each shift, ensuring accuracy and reporting any discrepancies to operations management immediately
Administer medications accurately and on schedule in accordance with physician orders documented in the company’s EMR or EHR system
Follow the Company’s Medication Observation policy at all times
Report medication refusals, errors, or other medication-related concerns in accordance with organizational protocols
Attend all ongoing assigned medication administration training

Documentation & Compliance

Complete shift reports documenting client observations, incidents, and interventions during assigned shifts
Report and document all client-related incidents in accordance with organizational policies, procedures, and reporting timelines
Complete required compliance documentation related to client admissions, discharges, and Leaves of Absence
Complete all required compliance and regulatory training assigned through the company’s designated systems (Kipu, Paycom, etc.) within established deadlines assigned by manager and HR
Conform to all necessary quality assurance standards as well as all infection control protocols
Audit face-to-face rounds as applicable

Client Transportation

Safely transport and accompany clients to and from the clinic as well as to any additional outside appointments
Provide supervision to clients during outside appointments depending on clinical recommendation
Inspect company vehicles upon shift change with previously scheduled team members

Communication & Collaboration

Participate in shift exchange to communicate open tasks and client updates
Communicate significant client concerns, behavioral changes, and safety issues promptly to the appropriate clinical or operations leadership

Ongoing Required Trainings

Attend and actively participate in all mandatory monthly operations training calls
Attend mandatory quarterly clinical trainings
Participate in specialized trainings provided by the Training Supervisor as assigned

General Responsibilities

Assist in maintaining a clean and organized clinic, tech office, and house by disinfecting common areas and restocking and reorganizing supplies
Conduct weekly urine analysis (UA) for clients, ensuring timely collection and reporting of results
Monitor inventory levels weekly, restock supplies, and communicate supply needs promptly

Other duties as assigned

● Must be able to pass a LiveScan background check
