Full Job Description

Job Overview
Our Dallas-based Foster Care Child Placing Agency is seeking a dedicated Case Manager to join our team. The Case Manager will be responsible for providing direct support and guidance to foster children and families, while serving an intergral member of a visionary team which fills the mission of the non-profit organization.

Aftercare case management is a collaborative process that assesses needs, creates a plan, coordinates services, monitors progress, and adjusts support over time. This includes medication management, followup appointments, rehabilitation coordination, and ensuring continuity of care.

Duties
- Collaborate with treatment teams to develop and ensure implementation of indvidual strengths based service plans,
- Coordinate services and resources to meet clients' needs effectively,
- Advocate for clients within the community and with relevant agencies,
- Maintain accurate and up-to-date case notes and documentation,

-Provide foster parents with support and resources,

-Maintain compliance with minimum standards as relevant to the position
- Provide crisis intervention when necessary

- Transitional Meeting and coordination of continued services.

Requirements
- Bachelor's Degree in Social Services or related field;

-Ability to clear a background check and employment drug screen per minimum standard;

-current vehichle liability insurance and Driver's License;

-Prior experience with the foster are population;
- Knowledge of childcare practices and social work principles;
- Ability to work effectively with diverse populations;
- Understanding of child protective services and related regulations;
- Strong communication and interpersonal skills;
- Willingness to accept feedback and adapt to changing situations;

Job Type: Full-time

Pay: From $45,000.00 per year
