Clinical Infusion Intake Pharmacist / Pharmacy Technician

Landmark Infusion is seeking an Infusion Intake Pharmacist and/or Infusion Intake Pharmacy Technician to join our growing team. In this role, you will coordinate patients starting home infusion therapies with Landmark Infusion, working closely with internal teams — including Reimbursement Specialists and Client Relations Directors — to keep every referral moving efficiently and accurately. You will also enter new patient orders into our order processing system, CareTend.

The ideal candidate brings a strong sense of urgency, excellent organizational skills, and a high level of customer service, with consistent follow-through for both patients and referral partners.

JOB RESPONSIBILITIES

Provide excellent customer service to patients, families, caregivers, and other healthcare providers, with a focus on patient-centered care and safety.
Monitor and update infusion queues with referral status to ensure patients are tracked for timely delivery from clinic or acute care settings to home.
Communicate heavily by phone with patients, maintaining a high level of customer service throughout.
Work collaboratively with the Sales Team and patient Educators to ensure efficient discharge and start of care.
Educate patients and caregivers on Landmark's infusion pharmacy and skilled nursing services.
Provide patient financial counseling to set payment expectations, collect patient responsibility, and establish payment plans as needed.
Review patient accounts to identify needed medical documentation, and notify the Sales Team when additional information is required.
Verify physician information at the initiation of the referral (NPI, PECOS, licensure).
Assist in coordinating simultaneous services provided by the company, including home infusion pharmacy and skilled nursing services.
Coordinate and establish external skilled nursing services as needed.
Maintain confidentiality of patient and proprietary information, and follow all applicable federal, state, and regulatory guidelines.
Follow company policies and procedures, and pursue ongoing professional growth through continuing education and process improvement initiatives.
Assist other departments and perform other duties as assigned.

Additional Responsibilities — Pharmacist

Perform clinical review of new referrals, including therapy appropriateness, dosing, and drug therapy assessment prior to order entry.
Verify and clinically validate physician orders for accuracy, safety, and compliance with infusion protocols.
Provide clinical consultation to patients, caregivers, and referral sources regarding infusion medications, potential interactions, and expected therapy outcomes.
Collaborate with prescribers on therapy modifications, clarifications, or clinical concerns identified during intake.
Serve as a clinical resource for the Intake and Sales teams on complex or high-risk referrals.

EDUCATION/EXPERIENCE

Pharmacy Technician

Must have a valid Pharmacy Technician license in the state of Texas.
High school diploma or equivalent required.

Pharmacist

Must have a valid Pharmacist license in the state of Texas, in good standing.
Doctor of Pharmacy (PharmD) or equivalent pharmacy degree required.
Experience in home infusion, specialty pharmacy, or clinical pharmacy practice preferred.

Shared Requirements (Both Roles)

Experience working in previous infusion pharmacies is preferred.
Experience working in a position with high call volume is needed.
Experience working in a position that involves collecting payments is preferred.
Must have a positive, can-do attitude and strong customer service skills.
Must have ample phone call experience.
Working knowledge of medical terminology.
Bilingual is a plus.
Computer proficiency – MS Office.
Excellent verbal and written communication skills.
Meets all productivity requirements.
Ability to work independently within a team environment.
Prolonged sitting and some standing required; occasional need to lift, pull, carry, and push items weighing up to 50 pounds.
Works directly with our Revenue Cycle Management team.
Experience with Office, Availity, CareTend, and Citus is a plus.
