Full Job Description

POSITION SUMMARY

Case Management and Transitional Housing with homeless youth, requiring knowledge of and experience in Mental Health, Addictive Disorders, Trauma Informed Care and Benefits Eligibility, especially as it relates to the homeless.

The position involves engaging in and building relationships with youth in the community who might be on the streets, couch surfing, and in encampments or abandoning buildings. The Youth YHDP Transitional Housing Case Manager will engage and coordinate temporary housing placements for youth experiencing unsheltered homelessness. This position will be working closely with the COC lead agency and internal outreach and crisis teams. This position will also partner with other youth outreach programs in Dallas County, including the ISDs, police & hospitals, as well as partner non-profits. Case Manager will be responsible for placing eligible youth experiencing homelessness into temporary shelter while engaging in rapid rehousing efforts.

QUALIFICATIONS

Education: Bachelor’s degree; Social Work or related field preferred.

Experience: One (1) year of experience in case management working with homeless population preferred; experience working with at-risk youth or youth with foster care history.

Licensure/Certification: None required.Current Texas Social Work license preferred.

Additional Requirements: Must be proficient in MS Word, Excel, and Outlook. Must be able to utilize a case tracking system. Must be available to work Monday-Friday 9am-5:30pm and some evenings as needed. Must have valid driver’s license and reliable transportation. Must be able to travel within assigned regions daily. Must be willing and able to meet potential clients within the community, including shelters and/or encampments.

PRIMARY RESPONSIBILITIES

This section describes the primary responsibilities of an employee in this position. Please note: The primary responsibilities of this position are not limited to this list.

1. Engage youth experiencing homelessness who are unsheltered and engage in rapid rehousing programs.

2. Initiate and manage client placement in temporary housing locations.

3. Responsible for conducting intake and assessment, both on the streets, in the office and at other organizations in the community, and developing individualized, short- & long-term goal plans.

4. Provide intensive case management services and warp around services to participants using a trauma informed approach.

5. Provide crisis management and intervention, utilizing de-escalation techniques.

6. Conduct daily site visits ensuring safety and compliance with program policy.

7. Utilize evidence-based practices, such as motivational interviewing, trauma informed care, and harm reduction.

8. Network and partner with community agencies for the purpose of obtaining information & coordinating service referrals.

9. Work closely with other Case Managers and Outreach workers on other teams.

10. Identify barriers to housing, such as treatment for substance abuse and mental health services or legal documents.

11. Link youth with all resources that are needed, both within and outside of TRAC.

12. Interface with the Dallas County Independent School District, Dallas Police Department, DART, and other local nonprofit youth outreach programs to facilitate services for current homeless youth.

13. Help to resolve issues and develop solutions for continued success.

14. Facilitate Life Skills classes, as needed.

15. Maintain current documentation and records of encounters, assessments, and referrals, as well as other statistical data required by the program, in active databases (such as ETO, HMIS and Columbus).

16. Must continually seek out and become familiar with community resources for homeless participants and help procure added resources.

17. Attendance at all community meetings related to homelessness, particularly youth services, in Dallas County.

18. Present for staff meetings, team meetings, All Staff meetings, and supervision.

19. Other Duties as assigned.

9. Ability to reach, turn and twist above and below the waist daily to retrieve files or supplies.

10. Ability to stand and walk daily to retrieve forms/supplies for other departments.

11. Ability to spend 25% of working time outside in temperature varying from 0oF to 110oF.

12. Ability to spend 75% of working time in environment of continuous moderate voices and office machine noise typical for business office atmosphere.

13. Ability to manage a variety of repetitive tasks.

14. Ability to work outside for much of the workday.

15. Ability to collaborate with participants who may be mentally ill or inebriated.

16. Ability to work evening hours and weekends as needed.
