Full Job Description

RESPONSIBLE TO: Director of Nurses

RESPONSIBILITIES:

The primary purpose of the Assistant Director of Nurses is to assist the DON in oversight of resident care in the community and assist with providing direction to the nursing teammates.

QUALIFICATIONS;

1. Must be a Registered Nurse (RN) or Licensed Vocational Nurse (LVN) in good standing and currently licensed in the state of Texas.

2. Be in good standing on Nurse Aide Registry and be able to fulfill all qualifications of a Nursing Assistant.

3. Preferred one to two years of experience in either Assisted Living or Long Term Care nursing management.

4. Preferred current Assisted Living Manager Certification (24-hour course)

5. Ability to understand, remember and carry out oral and written instructions in English.

6. Ability to communicate effectively both verbally and in written format with residents, families, other care providers and supervisors.

7. Excellent computer skills, ability to utilize electronic documentation.

8. Must be able to relate positively and favorably to residents and families and to work cooperatively with other employees.

9. Must be capable of maintaining regular attendance.

10. Knowledgeable of Teresa’s House Mission, Vision and Values. Conducts self in a manager that is congruent with the Mission, Vison and Values of Teresa’s House.

11. Ability to safely care for residents in a patient and respectful manner.

12. Ability to complete assignments while having constant interruptions.

13. Knowledge of Resident Rights per Assisted Living regulations.

14. Must be able to identify issues to be confronted or otherwise resolved pertaining to the dynamics of residency.

15. Must be capable of performing the Essential Job Functions on this job, with or without reasonable accommodation.

ESSENTIAL FUNCTIONS:

1. Assume DON responsibility in the absence of DON.

2. Responsible for monitoring resident weights and communicating concerns to DON, MD/NP, resident and responsible parties. Implement interventions determined to be best for the resident to maintain

3. Assist DON with screening, interviewing, selecting scheduling, supervising, inservicing, disciplining, evaluating and dismissing nursing employees in accordance with Teresa’s House policies and procedures and in consultation with Executive Director.

4. Assist DON with new and return admissions to Teresa’s House, ensuring all components of admission are completed, i.e., assessments, weights, vitals, service plan, MD orders, consents, 24-hour nursing form, care guides…

5. Assists with admissions as needed including giving tours, handing out brochures, answering questions, and distributing and collecting admission paperwork.

6. Responsible for medication carts for Teresa’s House, including reviewing MAR’s, ensuring medications are ordered and received. Assist DON with monthly resident counseling’s for those residents that self-administer medications and reviews pharmacy audits and addresses all issues. Administers medications as necessary or required.

7. Assist DON by reviewing monthly orders for accuracy. Medications have corresponding diagnosis; medications requiring monitoring (i.e., B/Ps with ranges to notify MD, pulse checks with ranges to notify MD, Blood sugar checks to notify MD…)

8. Makes rounds with MD/NP as needed.

9. Assist DON with obtaining psychotropic consents per regulations.

10. Ensure all labs and x-rays are provided and results communicated with MD and responsible party.

11. Conduct chart audits per assignment.

12. Assist DON with promptly investigating and following-up on all incident reports. Participates in daily incident report meetings.

13. Ensure all forms are completed and filed per policy: Daily assignment sheets with staff signature, daily temperature logs, food temperature logs, incident reports…

14. Reviews daily 24 forms, ensure nurses are documenting per policy as well as communicating COC with MD and responsible party.

15. Moves quickly and efficiently to all areas of Teresa’s House in order to respond to emergencies.

16. Participates in nursing on-call rotation and ensure adequate staffing,

17. Conducts nursing supply inventory weekly. Maintains a par level and provides DON with supply list weekly to ensure all supplies are available to meet resident’s needs.

18. Oversees the dining room process – ensures all residents have appropriate utensils and other supportive devices to assist in eating

19. Assists DON with monitoring Home Health and Hospice programs. Reviews plans of care are incorporated into resident service plan.

20. Encouraging residents to maintain their highest possible independence, while maintaining their resident’s rights.

21. Participate in teaching and promoting self-care for residents.

22. Participate in emergency fire or disaster drills and other in-service training as scheduled.

23. Participate in departmental meetings and family meetings as directed.

24. Follow Universal Safety Precautions.

OTHER DUTIES:

1. Perform other duties as assigned by the Executive Director
