Job Summary

The Transitions of Care Pharmacy Support role is an essential member of the pharmacy team responsible for supporting clinical services that assist patients during transitions between care settings. This position performs comprehensive medication reconciliation for admitted patients by gathering accurate medication histories and documenting findings in the electronic medical record for clinical review. The role also supports transitions-of-care initiatives by coordinating bedside delivery of discharge medications through the Medication-to-Bed (M2B) program, working collaboratively with pharmacists, nurses, physicians, and the outpatient pharmacy to ensure timely filling, delivery, and review of medications before discharge. The Transitions of Care Pharmacy Support team member enhances continuity of care, reduces medication-related errors, and contributes to improved patient outcomes through effective communication and coordination.

Essential Functions

JOB SPECIFIC COMPETENCIES:

Demonstrates the required job specific technical/professional knowledge, skills and abilities. *

Medication Reconciliation
- Conduct structured patient, family, and caregiver interviews to obtain accurate home medication histories at the time of admission.
- Verify medication information using external sources such as pharmacy records, outpatient EMRs, previous hospital discharges, and medication containers when available.
- Accurately document all home medication information in the electronic medical record for provider and pharmacist review.
- Identify discrepancies, missing information, or potential safety issues and escalate concerns to the covering pharmacist.
- Collaborate with the clinical pharmacy team to support transitions-of-care workflows for high-risk or complex patients.
Medication-to-Bed (Meds-to-Beds) Program
- Demonstrates the ability to clearly explain the Meds-to-Beds service to eligible patients, including program limitations, payment requirements, insurance needs, and follow-up expectations, while addressing patient questions using approved talking points.
- Completes the M2B intake process accurately by entering patient, insurance, and enrollment information into the necessary systems/portals.
- Effectively communicates with pharmacists, nurses, hospitalists, and the outpatient pharmacy to coordinate medication delivery, follow up on missing information, and ensure timely processing of prescriptions and copay information.
- Efficiently reviews copay amounts with patients, collects credit card information using the designated payment portal, and documents acceptance or rejection of copays while escalating issues appropriately when medication alternatives are needed.
- Consistently uses secure chat, email, and Epic handoff tools to communicate patient enrollment status, outstanding tasks, rejected medications, and required provider actions, ensuring clear, accurate, and timely transitions between team members and shifts.

Qualifications

High School Required or
GED Required
Two years pharmacy or patient-focused healthcare experience preferred. Preferred
Should have excellent knowledge of drug names and medical abbreviations.
PHARMT Pharmacy Technician - Pharmacy Technician Certification Board Required
Registered Pharmacy Technician - New York State Board of Pharmacy within 90 Days
