POSITION SUMMARY:
The Corrections Navigator provides comprehensive corrections navigation services including patient advocacy and care coordination including navigation of medical, social, community, legal, financial, and other needed services to People Living with HIV/AIDS (PLWH) scheduled to be released from incarceration.
Work involves providing supportive services to promote participant access to case management, food, transportation, housing, medical care and increase participant self-sufficiency; performing client-based and administrative tasks involved in the maintenance of a caseload of Ryan White program participants; providing interim support for walk-in participants; and providing crisis intervention and suicide risk assessment and follow up. The principal duties are performed in a social service environment where participants receive supportive services related to case management.
RESPONSIBILITIES AND DUTIES:
- Pre-release services: Complete pre-release linkage services to incarcerated PLWH in TN prisons and jails to prepare the individual for transition from incarceration to the community with continued access to medication and HIV-related support services post release.
- Corrections navigation linkage to medical care: Link PLWH scheduled to be released from incarceration to HIV medical care and treatment.
- Corrections navigation support service referrals: Link PLWH scheduled to be released from incarceration to support service referrals.
- Corrections navigation follow-up: Reassess PLWH for support and medical services to ensure continuity of care six months post incarceration.
- Develop individualized care plans: Assist participants in development of a strength-based/solution-focused individualized goal and action plan that promotes self-sufficiency.
- Assess and address barriers: Identify participant strengths and barriers to stability; aid in reducing barriers by connecting participants to resources and services.
- Provide referrals: Offer information and referral assistance to available support from social service agencies and community programs.
- Conduct Ryan White case management: Provide intakes, assessments, and ongoing case management for a caseload of participants in the Ryan White program.
- Maintain participant records: Keep accurate and complete records for all participants, adhering to Ryan White Part B Standards of Care and the Medical Case Management Manual. Create, update, and manage records in the electronic medical record, RWES, and other agency documentation systems.
- Assist with budgeting: Aid in developing personal budgets with participants, offering budget counseling and education.
- Insurance assistance: Help with insurance enrollments and various insurance assistance programs.
- Provide interim support for walk-ins: Serve as the point of contact for walk-in clients whose assigned case manager is unavailable. Offer immediate assistance by providing information on community resources, distributing emergency supplies (such as food from the pantry), and assisting with urgent tasks such as contacting insurance providers. Ensure the client's immediate needs are addressed and document the interaction for follow-up by the assigned case manager.
- Conduct crisis intervention and suicide risk assessment: Provide immediate crisis intervention for clients experiencing emotional or psychological distress. Perform thorough suicide risk assessments and facilitate referrals to mental health services or emergency care as needed. Assist participants in developing an agreed-upon safety plan to mitigate the risk for suicide. Follow up with participants 1-2 days after crisis intervention to assess safety and provide additional support if needed.
- Participate in integrated care meetings: Attend huddle daily as well as weekly treatment team meetings to support CHN’s holistic care model.
- Document services provided: Maintain daily documentation of services provided to participants, notating thoroughly to ensure continuity of care. Enter Ryan White service units accurately in CAREWare.
- Handle administrative tasks: Work within the electronic medical record, including scanning and uploading documents, and creating or updating electronic charts.
- Timely communication: Respond promptly to calls, emails, and other communications.
- Customer experience duties: As part of a team, assist in front desk duties including answering calls.
- Complete other tasks as deemed appropriate.
JOB QUALIFICATIONS AND SKILLS:
- Excellent communication skills, particularly listening, mediation, and writing skills.
- Possess strong organizational skills with ability to meet a demanding workload.
- Detail oriented to complete requirements of files and contract compliance.
- Creative thinker/adaptive personality.
- Fluency in the English language is required. Spanish speaking ability is an added asset but is not required.
- Experience with computers and knowledge of Microsoft Office.
- Sensitivity to cultural and socioeconomic characteristics of population served.

- A conviction about the capacity of people to grow and change.
- The ability to establish and set appropriate limits with persons served to help them gain skills and confidence.

People with lived experience of substance use, incarceration, homelessness, and sex work; people of color; members of the LGBTQIA+ community; and, people living with HIV and/or hepatitis C are encouraged to apply.
- Experience providing case management to underserved communities.
- Minimum Education Requirements: Bachelor’s Degree in Social Work or other related field.
