Full Job Description

RN Case Manager | Remote | Contract

Walker Healthforce is seeking an RN Case Manager with 3+ years of clinical nursing experience to provide comprehensive care coordination and case management services for health plan members. This is a Contract opportunity.

Start Date: ASAP

Location: Remote local to MI

Hours/Schedule: Monday–Friday, 40 hours

Worker Type: W2

Core Requirements

3+ years of clinical nursing experience in acute, post-acute, or community care.
Current, active, unrestricted Michigan RN license.
Strong care coordination, assessment, and communication skills.
Ability to manage members telephonically and through digital communication platforms.

We Consider It A Bonus If You Also Have

Bachelor's Degree in Nursing.
Case management experience in a managed care environment.
Certified Case Manager (CCM).
Certified Chronic Care Professional (CCP).
Experience with telephonic and digital case management.

Job Responsibilities

Coordinate care across multidisciplinary teams.
Develop and manage individualized care plans.
Assess member needs and identify care gaps.
Collaborate with providers, caregivers, and community resources.
Educate members and caregivers regarding care plans.
Monitor member progress and adjust care plans as appropriate.
Maintain accurate documentation.
