AbilityHub+ is seeking a compassionate, organized, and dependable Case Manager to provide case management services and support for children and youth in state custody. This position plays an essential role in coordinating services, maintaining required documentation, advocating for each child’s needs, and collaborating with state agencies, schools, healthcare providers, families, and other members of the child’s treatment team.The ideal candidate has experience working with children and adolescents, child welfare, foster care, behavioral health, social services, or youth in state custody and understands the importance of trauma-informed, professional, and consistent care.Key Responsibilities
- Manage an assigned caseload of children and youth in state custody.
- Develop, implement, monitor, and update individualized service and case plans based on each child’s needs and goals.
- Maintain regular contact with assigned youth and monitor their safety, well-being, placement stability, and overall progress.
- Coordinate medical, behavioral health, dental, educational, recreational, and other necessary services.
- Attend and participate in treatment team meetings, case conferences, school meetings, court-related meetings, discharge planning, and other required meetings.
- Communicate regularly with state agency representatives, social workers, supervisors, healthcare providers, schools, therapists, and other members of the treatment team.
- Advocate for the child’s educational, medical, behavioral, emotional, and social needs.
- Assist with coordinating school enrollment, IEP meetings, educational services, appointments, transportation needs, and other required resources.
- Complete case notes, progress reports, incident follow-ups, assessments, service plans, and other required documentation accurately and within established deadlines.
- Maintain organized and complete case files in accordance with agency and state requirements.
- Monitor compliance with each child’s placement and treatment plan.
- Identify changes in behavior, safety concerns, placement concerns, or other significant issues and immediately communicate them to appropriate leadership and state representatives.
- Participate in crisis intervention and assist with coordinating appropriate responses when a youth experiences a behavioral or emotional crisis.
- Support youth in developing independent living, communication, coping, decision-making, and life skills when developmentally appropriate.
- Help coordinate family contact and visitation as authorized by the child’s case plan and state agency.
- Assist with admission, placement transitions, discharge planning, and coordination of services when youth enter or leave the program.
- Maintain professional boundaries and protect the confidentiality and privacy of all youth.
- Follow mandatory reporting requirements and immediately report suspected abuse, neglect, exploitation, or other safety concerns.
- Work collaboratively with direct-care staff and clinical personnel to ensure consistency between the child’s case plan and daily services.
- Ensure services are delivered using a trauma-informed, child-centered approach.
- Maintain compliance with AbilityHub+ policies, state requirements, confidentiality standards, and all applicable regulations.
- Perform additional case management and program-related duties as assigned.
Minimum Qualifications
- Bachelor’s degree in Social Work, Psychology, Sociology, Counseling, Human Services, Criminal Justice, Child and Family Studies, or a related human-services field required.
- Previous professional experience working with children or adolescents is required
- Experience in child welfare, foster care, residential care, behavioral health, social services, or case management is highly preferred.
- Ability to work effectively with children who have experienced trauma, abuse, neglect, loss, behavioral challenges, or placement disruptions.
- Strong written and verbal communication skills.
- Excellent documentation, organization, and time-management skills.
- Ability to manage multiple cases, appointments, deadlines, and priorities.
- Ability to communicate professionally with state agencies, families, schools, healthcare providers, and multidisciplinary teams.
- Must maintain confidentiality and professional boundaries at all times.
- Must be dependable, professional, and able to respond appropriately to sensitive or crisis situations.
- Must successfully complete all required background checks, screenings, and agency-required training.
- Valid driver’s license and reliable transportation required.
Preferred Qualifications
- Previous experience working directly with children in state custody.
- Knowledge of child welfare systems and trauma-informed care.
- Experience developing or monitoring individualized service plans.
- Experience working with multidisciplinary treatment teams and state child welfare agencies.
- CPI or other approved crisis-prevention/de-escalation training is required.
- CPR required
