Full Job Description

Job Summary

Pareto Clinic is a physician-led family medicine practice in Jacksonville, Florida. We are hiring an experienced Nurse Practitioner or Physician Assistant to provide outpatient primary care for adults, women, children, seniors, and families.

This role is ideal for a provider with at least one year of outpatient primary care, family medicine, pediatrics, women’s health, urgent care, or similar experience who wants to practice meaningful, modern primary care in a team-supported environment.

This is real primary care — designed for where medicine is going.

We believe primary care is one of the most meaningful ways to serve a community. We also believe the traditional model of primary care is no longer sustainable when clinicians are unsupported, overburdened, and expected to complete the workday after the workday ends.

Pareto Clinic was built to do primary care differently: with team-based workflows, technology-enabled care, quality-focused support, telemedicine, care coordination, and a commitment to protecting providers from unnecessary burnout.

What This Role Looks Like

This is outpatient family medicine in a modern, growing practice. The provider will see patients face-to-face and may also provide telemedicine care. You will care for adults, women, children, seniors, and families, with support from a clinical and administrative team.

The goal is not simply to see more patients. The goal is to deliver good care, close charts timely, improve outcomes, support access, and build long-term patient relationships.

Clinical Responsibilities

Provide outpatient primary care for adults, women, children, seniors, and families.
Evaluate and manage acute concerns, chronic conditions, preventive care needs, and follow-up visits.
Manage common primary care conditions such as hypertension, diabetes, asthma/COPD, obesity, anxiety/depression, infections, medication refills, and preventive screenings.
Provide women’s health services within family medicine scope.
Support pediatric and adolescent primary care, including well-child visits and common acute concerns.
Review labs, imaging, consult notes, hospital records, and diagnostic results.
Develop individualized care plans and provide patient education focused on prevention, lifestyle, medication adherence, and chronic disease management.
Participate in telemedicine visits and technology-enabled care when appropriate.
Complete accurate and timely documentation in the electronic health record.
Collaborate with physicians, providers, medical assistants, care coordinators, referral staff, and quality team members.
Support care gap closure, HEDIS measures, preventive screenings, risk adjustment, chronic care management, and value-based care initiatives.

A Team-Based Model

You are not expected to do this alone. Our providers work with a team that supports visit preparation, care coordination, referrals, patient outreach, quality measures, documentation workflows, and follow-up.

We are intentionally building a practice where providers can complete high-quality documentation, close charts timely, and avoid a culture of excessive after-hours charting.

A Future-Facing Practice

AI is already changing medicine. At Pareto Clinic, we use technology thoughtfully to support documentation, care coordination, patient access, quality improvement, and team efficiency.

AI does not replace clinical judgment. It supports the provider so the provider can focus on the patient, the plan, and the quality of care.

Qualifications

Active Florida APRN or Physician Assistant license.
At least one year of outpatient primary care, family medicine, internal medicine, pediatrics, women’s health, urgent care, or comparable experience preferred.
Comfort caring for adults, women, children, seniors, and families.
Strong clinical judgment and documentation habits.
Ability to work collaboratively with physicians, providers, medical assistants, and care team members.
Commitment to timely chart completion and patient follow-through.
Professional, dependable, compassionate, and team-oriented.

Preferred Experience

AthenaOne or Athenahealth experience.
Telemedicine experience.
Medicare, Medicaid, HEDIS, quality measures, risk adjustment, chronic care management, or value-based care experience.
Experience with preventive care, pediatric primary care, women’s health, and chronic disease management.
Interest in teaching, mentoring, process improvement, or helping shape modern primary care workflows.

Compensation

Full-time compensation is expected to range from $105,000 to $125,000 annually, depending on experience, schedule, clinical fit, and role responsibilities. Part-time compensation may be offered on an hourly or prorated basis.

Additional opportunities may be available based on quality performance, productivity, teaching, leadership, care coordination, and contribution to practice growth.
