Full Job Description

Position Summary

The Certified Home Health Agency (CHHA) Department is seeking a compassionate, experienced, and motivated Per Diem Registered Nurse (RN) to provide high-quality nursing services to patients throughout the New York Metropolitan Area. The CHHA Field RN is responsible for delivering comprehensive skilled nursing care, coordinating multidisciplinary services, completing required clinical assessments and documentation, and ensuring compliance with all New York State Department of Health (NYSDOH), Medicare, and agency regulations.

Working collaboratively with physicians, therapists, home health aides, patients, and caregivers, the CHHA Field RN develops, implements, and evaluates individualized plans of care designed to promote optimal patient outcomes.

Essential Duties and Responsibilities

Patient Care & Clinical Services

Provide and coordinate skilled nursing services in accordance with physician orders, agency policies, and payer guidelines.
Deliver cost-effective, high-quality patient care that is medically necessary and reimbursable under the patient's insurance plan.
Verify patient insurance information and confirm the patient's insurance identification during home visits.
Accept case assignments based on patient needs, regardless of geographic location within the assigned service area.
Conduct initial assessments, follow-up visits, recertifications, resumptions of care, and other CHHA-required nursing visits.
Travel to patients' homes and other approved care settings to provide direct nursing services.
Admit patients to the CHHA program in a timely manner while ensuring all admission requirements are completed accurately.

Assessment, Documentation & Care Planning

Complete OASIS assessments in person, ensuring accurate representation of each patient's clinical and functional status.
Utilize appropriate diagnosis coding to accurately reflect patient acuity.
Collaborate with the Director of Patient Services (DPS) and attending physician to determine the appropriate primary diagnosis.
Develop, implement, and update individualized Plans of Care (POC) in collaboration with the interdisciplinary care team, patient, and family.
Monitor patient progress, evaluate the effectiveness of interventions, and revise care plans as appropriate.
Maintain timely, accurate, and complete clinical documentation in accordance with agency, Medicare, and regulatory standards.
Document all patient care activities, assessments, supervisory visits, and communications thoroughly and accurately.

Care Coordination

Serve as the clinical coordinator of the interdisciplinary team responsible for the patient's care.
Communicate regularly with physicians, physical therapists, occupational therapists, speech therapists, social workers, and other healthcare professionals to ensure coordinated service delivery.
Report patient progress to the interdisciplinary team and assist in establishing anticipated discharge goals and timelines.
Notify physicians and appropriate team members promptly of significant changes in patient condition or care needs.

Patient & Family Education

Educate patients and caregivers regarding disease processes, medications, treatment plans, self-care techniques, safety measures, and preventive health practices.
Promote patient independence while supporting the achievement of established health goals.

Home Health Aide (HHA) Supervision

Supervise Home Health Aides (HHAs) in accordance with CHHA, NYSDOH, and agency requirements.
Complete and document initial and bi-weekly supervisory visits as required.
Maintain and review the Home Health Aide Plan of Care with the HHA, patient, and family.
Communicate regularly with HHAs regarding patient goals, progress, and changes to the interdisciplinary Plan of Care.

Case Management

Manage an assigned patient caseload within the designated service territory.
Coordinate patient services to ensure continuity of care throughout the episode of care.
Plan, facilitate, and document patient discharges from the CHHA program.

Required Knowledge, Skills & Abilities

Thorough knowledge of Certified Home Health Agency (CHHA) regulations, standards, and practices.
Working knowledge of OASIS assessment requirements and documentation.
Strong clinical assessment, critical thinking, and decision-making skills.
Excellent communication, interpersonal, and organizational skills.
Ability to work independently while collaborating effectively within an interdisciplinary healthcare team.
Proficiency in electronic health record (EHR) documentation systems.
Ability to manage multiple patient cases while maintaining accuracy and attention to detail.

Minimum Qualifications

Current New York State Registered Professional Nurse (RN) license in good standing.
Graduate of an accredited school of nursing.
Minimum of one (1) year of Certified Home Health Agency (CHHA) nursing experience preferred.
Valid driver's license and reliable transportation, or the ability to travel throughout the assigned service area.
Ability to meet all agency health, background screening, and credentialing requirements.

Working Conditions

Field-based position requiring frequent travel throughout the New York Metropolitan Area.
Regular exposure to home and community care environments.
Must be able to stand, walk, bend, lift, and perform the physical requirements necessary to provide direct patient care.
