Full Job Description

Title: PRN Hospice Registered Nurse - RN

Job Status: PRN

Job Location: Jasper, AL

Office: Access Hospice

POSITION SUMMARY: The Registered Nurse plans, organizes, directs and coordinates hospice care services. The RN demonstrates experience in nursing, with an emphasis on community health education. The professional nurse utilizes community resources to provide services that meet the needs of patients, families, and caregivers within their homes and communities.

ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

Completes initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
Provides a complete physical assessment and history of current and previous patient illness(es).
Provides professional nursing care by utilizing all elements of nursing process.
Assesses and evaluates patient’s status by:
- Writing and initiating plan of care.
- Regularly re-evaluating patient, family, and caregiver needs.
- Participating in revising the plan of care as necessary.
Uses health assessment data to determine nursing diagnosis.
Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
Includes patient and family in the care planning process.
Initiates appropriate preventive and rehabilitative nursing procedures.
Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan.
Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
Provides case management services and coordinates patient care for assigned caseload.
Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given.
Records pain and symptom management changes/outcomes as appropriate.
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
Communicates with community health-related persons to coordinate the care plan.
Teaches the patient, family, and caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients, families, and caregivers. Works in concert with the Interdisciplinary Group (IDG).
Provides and maintains a safe environment for the patient.
Assists the patient, family, caregiver, and other team members in providing continuity of care.
Works in cooperation with the family, caregiver, and hospice IDG to meet the emotional needs of the patient, family, and caregiver.
Attends IDG meetings.
Participates in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services are available.
Supervises ancillary personnel and delegates responsibilities when required.
Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
Fulfills the obligation of requested and/or accepted case assignments.
Actively participates in quality assessment performance improvement teams and activities.
Other appropriate services and special projects as assigned.

QUALIFICATIONS & REQUIREMENTS FOR THE POSITION:

Current RN licensure in state of practice.
At least two (2) years of experience working as a Registered Nurse in health care, hospice experience preferred.

Current CPR certification required.
Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.

PREFERRED KNOWLEDGE AND SKILLS:

Excellent time management and problem-solving skills.
Must have outstanding verbal and written communication skills.
Must be self-motivated and able to work independently while being collaborative and able to work as part of an Interdisciplinary Group.

SUPERVISORY RESPONSIBILITIES:

May supervise HHA, CNA, LPN, and/or LVN as needed.

Job may require heavy use of office equipment, including computer, tablet, and phone.
Must have the reasonable ability to stand, walk, sit, use hands to finger, handle, feel, reach, balance, see, talk, hear, and drive. Must be reasonably capable of lifting over thirty (30) pounds.
Significant portion (>50%) of daily assignments may require travel to client/resident/patient locations or other work sites, via car or public transportation.
Work schedule may occasionally require evening, night, holiday, or weekend assignments.
