Full Job Description

Position Title

Full-Time- RN Case Manager

Location:
Canton, GA/Jasper, GA

Position Overview

The Full-Time Hospice Registered Nurse (RN) provides compassionate, high-quality end-of-life care to patients in a variety of settings, including private homes, assisted living facilities, nursing homes, and inpatient units. The RN ensures patients’ comfort and dignity, supports families, and collaborates with an interdisciplinary team—physicians, social workers, chaplains, and other clinical staff—to create and carry out individualized care plans.

Key Responsibilities

Patient Assessment & Admission
Perform comprehensive nursing assessments of new hospice admissions, assessing patient health status, pain levels, and care needs.
Communicate hospice philosophy, benefits, and processes to patients and families.
Collaborate with the interdisciplinary team to determine eligibility and develop a care plan.
Care Plan Development & Implementation
Formulate individualized care plans focused on symptom management and patient comfort, in coordination with the attending physician and team.
Administer treatments, medications, and interventions based on the care plan, following all agency and regulatory requirements.
Symptom Management & Comfort Measures
Provide pain relief and symptom management through medication administration and ongoing monitoring.
Track patient progress and adjust treatments as necessary in consultation with the attending physician.
Educate patients and families on medication regimens, side effects, and non-pharmacological interventions.
Family & Caregiver Support
Serve as the primary point of contact for patients and families, delivering empathetic education regarding disease progression.
Teach caregivers essential skills for safe patient care and provide emotional support throughout the hospice journey.
Assist families with navigating available resources and make referrals when additional support is needed.
Interdisciplinary Team Collaboration
Attend and participate in interdisciplinary team meetings, sharing critical patient updates and care plan recommendations.
Maintain clear, consistent communication with other team members—physicians, social workers, chaplains, etc.—to provide holistic patient support.
Coordinate care transitions between settings (home, inpatient, skilled nursing) as necessary.
Documentation & Compliance
Maintain accurate, up-to-date patient records within the electronic health record (EHR) system.
Ensure all documentation meets agency standards, state and federal regulations, and Medicare hospice guidelines.
Support external and internal audits as needed.
On-Call & Emergency Response
Participate in on-call rotations to guarantee 24/7 patient access to hospice care.
Respond promptly to emergent symptoms or urgent patient needs, providing necessary interventions and coordinating with the care team.
Quality Improvement & Professional Growth
Contribute to quality assurance and performance improvement (QAPI) initiatives.
Pursue educational opportunities that enhance knowledge of hospice and palliative care.
Maintain professional licensure requirements through in-service trainings and other required courses.
Patient Advocacy
Advocate for patient rights, ensuring all care aligns with patient values, preferences, and dignity.
Facilitate discussions around advance directives, living wills, and end-of-life care goals.

Qualifications

Education & Licensure
Graduate from an accredited School of Nursing (Associate’s or Bachelor’s degree in Nursing).
Current, unrestricted Registered Nurse (RN) license in the State of Georgia.
Basic Life Support (BLS) certification preferred.
Experience
At least one year of experience in hospice, palliative care, home health, or a similar clinical field (preferred).
Experience with pain management and symptom control is highly beneficial.
Skills & Competencies
Strong clinical assessment and critical-thinking skills.
Outstanding communication abilities for clear patient/family education and interdisciplinary collaboration.
Demonstrated empathy, compassion, and professionalism when dealing with end-of-life care.
Familiarity with electronic medical record (EMR) systems.
Excellent organizational and time-management skills.
Personal Attributes
Dedicated to the hospice philosophy, committed to patient comfort and dignity.
Flexible in handling varied patient needs and changing workloads.
Team-oriented, with the ability to also work independently.

Working Conditions

Schedule: Full-time, typically Monday–Friday with occasional on-call responsibilities (including some nights, weekends, and holidays).
Travel: Regular local travel to patient homes and care facilities in the greater Atlanta area. Reliable transportation is required.
Physical Requirements: Ability to lift or transfer patients safely. Frequent standing, walking, and bending are part of daily duties.
